Primary biliary cirrhosis associated with brucellosis infection: A case report

Abstract

A 30-year-old woman was admitted to the hospital because of abdominal pain, intermittent fever and emesis. Primary biliary cirrhosis had been diagnosed two years earlier, and she had taken ursodeoxycholic acid 15 mg/kg/day. Her temperature was 39°C and the pulse was 70 per minute. Laboratory tests revealed leukopenia and thrombocytopenia. Liver enzymes and C-Reactive Protein levels were also high. An ultrasonography of the abdomen revealed splenomegaly and liver cirrhosis. The serum agglutination test for Brucella was reported as positive. She received a combination of brucellosis theraphy for 6 weeks. After completing curative therapy with ciprofloxacin and doxycycline, the patient has remained healthy, with no recurrence of fever or pain. Laboratory parameters remained normal.

Özet

30 yaşında kadın hasta karın ağrısı, aralıklı ateş ve kusma ile başvurdu. İki yıl önce primer biliyer siroz tanısını alan hasta 15 mg/kg/gün dozunda ursodeoksikolik asit kullanıyordu. Hastanın ateşi 39ºC ve nabzı 70/ dakika idi. Laboratuar incelemesinde lökopeni ve trombositopeni saptandı. Karaciğer enzimleri ve C- Reaktif Protein düzeyleri yüksekti. Batın ultrasonografisinde splenomegali ve karaciğer sirozu vardı. Brusella serum aglütinasyon testi pozitif idi. 6 hafta boyunca siprofloksasin ve doksisiklinden oluşan Brusella tedavisi aldı. Tedavi sonunda ateş, ağrı ve laboratuar değerleri eski haline döndü.
