An acute abdomen etiology in pregnancy: Torsion of a subserosal fibroid

Abstract

1 in 500-635 of pregnancies can be complicated by acute abdomen. The etiology of acute abdomen in pregnancy can be classified in two groups as obstetric and nonobsteric reasons. Ectopic grosses, ovarian masses, abortion, preterm birth, plasental abruption, uterine rupture, adnekxial torsion and fibroid degeneration are some obstetric reasons of acute abdomen. Only 2.6% of all pregnancies are affected by uterine fibroids. Fibroids may cause acute abdomen in pregnancy by inducing uterine torsion with the mass effect, by torsion of pedunculated fibroids and by degeneration of fibroids under oestrogenic stimulation. In this report, it is aimed to discuss an unusual case of acute abdomen in pregancy due to torsion of a pedunculous subserosal fibroid under the light of current literature.

Özet

Gebelikler 500 ila 635 olguda 1 akut batın ile komplike olabilmektedirler. Gebelik esnasında karşılaşılan akut batın nedenleri obstetrik ve obstetrik dışı nedenler şeklinde iki grup halinde değerlendirilebilir. Obstetrik akut batın nedenleri ektopik gebelik, ovarian kitleler, düşükler, erken doğum, ablasyo plasenta, uterin rüptür, adneksiyal torsiyon ve myom dejenerasyonu olarak sıralanabilir. Gebeliklerin ancak %2,6’sı myomlar tarafından komplike edilmektedir. Myomlar gebelikte kitle etkisi ile uterusu torsiyona uğratarak ya da pediküllü myomlar tek başına torsiyone olarak akut batın tablosuna yol açabilirler. Gebelikte nadir akut batın nedenlerinden biri olan torsiyone pediküllü subseröz myom olgusu, bu çalışma ile mevcut literatür ışığında tartışılmak istenmiştir.
