Thorax CT findings of malignant pleural mesothelioma caused by environmental asbestos exposure and its correlation with subtypes

Abstract
Purpose: Malignant pleural mesothelioma (MPM) is a tumor caused generally by asbestos exposure and diagnosis sometimes may be delayed 6-8 months later. Thorax tomoghraphy (Thorax CT) is a nonin-vasive radiologic method used in MPM. In our study, it was aimed to investigate the findings of CT and chest X-ray of patients and the relationship between the subtypes of the disease and findings. Methods: Thorax CT and chest X-ray of 166 patients diagnosed as MPM between January 1988-December 2003 were investigated. Of cases, 141 (% 85) had environmental asbestos exposure and mean age was 53 year. Results: Chest X-ray findings were detected as pleural effusion (PE), pleural thickening, loss of volume, mediastinal shift and mediastinal pleural thickening. Tomoghraphic findings were PE in 143 (% 86) patients, pleural thickening in 140 (% 84) patients,mediastinal pleural thickening in 96 (% 60) patients, interlobar fissure involvement in 80 (% 48) patients and volume loss in 78 (% 47) patients. Of patients being able to be classified, 68 was detected as epithelial, 27 as sarcomatous and 25 as mixed. CT findings of these types were compared. Mediastinal pleural, interlobar fissure and lung paranchymal involvement were found mostly in sarcomatous type. All of these three involvements were statistically high compared with the epithelial and mixed types. Mediastinal shift was also detected mostly in sarcomatous type. No statistical difference was detected between epithelial and mixed type. Conclusion: Thorax CT findings of MPM show varieties. MPM must be thought in patients having asbestos exposure if common tomoghraphic findings are detected. Additionally, mediastinal pleural involvement, interlobar fissure involvement and lung paranchyme involvement mostly detected in sarcomatous type may be the evidence of this type's being more invasive.

Amaç: Malign plevral mezotelyoma (MPM), genellikle asbest temasına bağlı olarak gelişen ve tanısında bazen 6-8 aylık gecikme ile saptanabilen bir tümördür. Toraks tomografisi (Toraks BT), MPM' da kullanılan noninvaziv radyolojik bir yöntemdir. Bu çalışmada hastaların Toraks BT ve PA akciğer grafisi bulgularının incelenmesi, bu bulguların hastalığın sub tipleriyle olan ilişkisinin araştırılması amaçlanmıştır. Gereç ve Yöntem: Ocak 1988-Aralık 2003 tarihleri arasında MPM tanılı 166 hastanın Toraks BT ve PA akciğer grafisi incelenmiştir. Olguların 141'inde (% 85) çevresel asbest teması mevcut olup ortalama yaş 53 olarak saptandı. Bulgular: Saptanan PA akciğer grafi bulguları plevral efüzyon (PE), plevral kalınlaşma (PK), volüm azalması, mediastinal yer değiştirme ve mediastinal PK idi. Tomografik olarak saptanan bulgular 143 (% 86) hastada PE, 140 (% 84) hastada PK, 96 (% 60) hastada mediastinal PK, 80 (% 48) hastada interlober fissür tutulumu ve 78 (% 47) hastada volüm azalmasıydı. Tip tayini yapılabilen hastaların 68'i epitelyal, 27'si sarkomatöz ve 25'i mikst tipti. Bu tiplerin Toraks BT bulguları karşılaştırıldı. Mediastinal plevra tutulumu, interlober fissür tutulumu ve akciğer parankim tutulumu en sık sarkomatöz tipte saptandı. Saptanan her üç tutulumda epitelyal ve mikst tiplere göre istatistiksel olarak anlamlı biçimde yüksekti. Mediastinal yer değiştirme (% 48) en sık sarkomatöz tipte rastlandı. Ancak mikst tip ile epitelial tip arasında Toraks BT bulguları yönünden istatistiksel bir farka rastlanmadı. Sonuç: MPM' de Toraks BT bulguları çok çeşitlidir. Asbest temaslılarda sık rastlanan tomografîk bulgular da eşlik ediyorsa MPM düşünülmelidir. Ayrıca mediastinal plevra tutulumu, interlober fissür tutulumu ve akciğer parankim tutulumunun sarkomatöz tipte daha fazla saptanması bu tipin daha invaziv olduğunun kanıtı olabilir.
