Evaluation of peritonitis attacks in patients under chronic ambulatory peritoneal dialysis

Abstract

Aim: This study was performed to evaluate the development of peritonitis in patients having continuous ambulatory peritoneal dialysis (CAPD). Methods: This study included all the patients received CAPD between January 1, 2009 and December 31, 2009. Patients’ file were evaluated retrospectively and the patients, who had peritonitis attacks were noted. The data obtained in these patients were evaluated. Results: Twenty-one patients had CAPD in this one year period and fifteen of these patients developed 31 peritonitis attacks. While in 16 attacks (51.6%), the patients had hypertension alone, both hypertension and diabetes was detected in patients in 6 peritonitis attacks (19.3%). There was positive culture in 24 of 31 (77.4%) peritoneal fluid samples. In the remaining 7 peritonitis attacks, diagnosis was established by biochemical and cytological evaluation of peritoneal fluid. Staphylococcus aureus was the most common pathogen (%41.7) among etiological agents. The patients’ CRP values and white blood cell counts during peritonitis episodes were found to be higher when compared with the values obtained prior to the attacks and this differences were statistically significant (p=0.0001). The peritonitis episodes were treated with parenteral antibiotics in 9 cases (29.03%) and with intraperitoneal antibiotics in the remaining 22 attacks (70.97%). While 25 peritonitis attacks could be totally treated with the above treatment, however, in the remaining 6 cases we failed to achieve infection control. Conclusion: Peritonitis is an important complication of CAPD, in which the common etiological agent was staphylococcus in our clinic. For the early diagnosis and proper management of peritonitis, CRP and white blood cell levels should be tested periodically.

Amaç: Çalışmada hastanemizde son bir yılda takip edilen sürekli ayaktan periton diyalizi hastalarında tespit edilen peritonit ataklarının değerlendirilmesi amaçlanmıştır. Yöntem: Çalışma retrospektif olarak planlanmış olup verileri hastaların dosya bilgileri taranarak elde edilmiştir. Bulgular: Çalışma periyodunda 21 hastaya SAPD uygulandığı tespit edildi. Bu hastaların 15inde (%48.4) 31 peritonit atağı geçirilmişti. Atak geçiren hastalarda eşlik eden komorbiditeler sıklık sırasına göre hipertansiyon (n:16, %51.6) ile hipertansiyon ve diyabet (n:6, %19.3) birlikteliği idi. Atakların 24ünde (%77.4) peritonit tanısı periton sıvısından yapılan kültürde etken üretilerek konulurken 7sinde (%22.6) tanı periton sıvısının biyokimyasal incelemesi ve hücre sayımı ile konulmuştu. Kültür pozitif olgularda en sık etken S aureus (%41.7) olarak tespit edildi. Hastaların atak öncesi ve atak sırasındaki CRP ve lökosit değerleri karşılaştırıldığında, atak sırasındaki değerler, atak öncesinden anlamlı düzeyde yüksek bulundu (p=0.0001, p=0.0001). Peritonit ataklarının 9u (%29.03) dammar içi yoldan, 22si (%70.97) intraperitoneal yoldan antibiyotik uygulanarak tedavi edildi. Tedavi sonucunda 25 atak (%80.65) şifa ile sonuçlandı. Sonuç: SAPD hastalarında peritonit ciddi bir problemdir. Peritonitlerin erken tanısında CRP ve lökosit değerlerinin yakından izlemi önemlidir. Çalışmamızda, peritonit etkeni olarak en sık saptanan mikroorganizma stafilokoklar olarak bulunmuştur.
