Abstract

Spontaneous torsion of the omentum majus is a rare cause of acute abdomen. Its clinical features may closely mimic acute appendicitis, acute cholecystitis, cecal diverticulitis or sigmoid diverticulitis. Torsion of the omentum can be either primary or secondary. It is rarely diagnosed preoperatively. The diagnosis is usually confirmed by laparotomy or laparoscopy. Treatment consists of resection of the affected portion of omentum. We report a case of spontaneous torsion of the omentum majus in a 57 year-old woman who admitted to the emergency department with a history of abdominal pain for two days. During the operation, twisted omentum majus was found on the right side and was removed. We suggest that spontaneous torsion of the omentum majus should be considered in the patients with acute abdomen.

Özet

Omentum majusun spontan torsiyonu akut batının nadir bir sebebidir. Klinik özellikleri akut apendisit, akut kolesistit, çekal divertikülit veya sigmoid divertikülite benzer. Omentumun torsiyonu ya primer yada sekonderdir. Operasyon öncesi nadiren tanı konulur. Tanı genellikle laparatomi yada laparoskopi ile kesinleştirilir. Tedavi omentumun etkilenen kısmının rezeksiyonundan ibarettir. iki günlük karın ağrısı hikayesi ile acil servise başvuran 57 yaşında kadın hastada omentum majusun spontan torsiyonunu rapor ettik. Operasyonda sağ tarafta dönmüs omentum tespit edildi ve rezeke edildi. Akut batınla gelen hastalarda omentum majusun spontan torsiyonunu akla getirilmelidir.
