Correlation of unenhanced helical CT and ultrasonography for the diagnosis of acute appendicitis

Abstract
Purpose: To evaluate the sensitivity, and specificity, of unenhanced CT and graded compression sonography in suspected appendicitis and to compare these results with surgery results. Methods: 52 patients who were suspected for acute appendicitis were evaluated with unenhanced spiral CT and US. On CT appendiceal luminal diameter, appendiceal wall thickening, presence of appendicolith, signs of periapendicular inflamation and perforation were evaluated. On US appendix lumen and compressibility, presence of appendicolith, presence of air in the lumen and perforation findings were evaluated. Final diagnosis was established by surgery in 33 patients and by follow-up inl9 patients. Results: The CT results were true positive in 27 patients, true negative in 20 patients, false positive in three patients and false negative in two patients. CT had a overall sensitivity of 93.1%, specificity 86.9%, positive predictive value 90%, negative predictive value 90.1%. The US results were true positive in 24 patients, true negative in 21 patients, false positive in 2 patients and false negative in 4 patients. The overall sensitivity, specificity, positive predictive value and negative predictive value of US were 85%, 91%, 93%, 84% respectively. Conclusion: Although the sensitivity and specificity of US and CT are close to each other. We suppose that US which is cost-effective, radiation free and easy to perform should be the first imaging modality. When US and clinical findings does not match CT should be used as problem solver.

Amaç: Bu çalışmanın amacı akut apandisit tanısında kontrastsız spiral bilgisayarlı tomografi (BT) ve ultrasonografi (US) bulgularının sensitivite ve spesifisite değerlerini araştırmak ve cerrahi sonuçları ile karşılaştırmaktır. Gereç ve Yöntem: Akut apandisit düşünülen 52 olguya oral ve rektal yoldan kontrast madde verilmeden spiral BT ve US incelemeleri yapıldı. BT'de apendiks çapı, duvarı, periapendiküler enflamasyon, apendikolit ve perforasyon bulgularının varlığı araştırıldı. US'de appendiks lümeninin çapı, kompresibilitesi, apendikolit varlığı, lümende hava varlığı ve perforasyon bulgularının varlığı araştırıldı. Olguların kesin tanısı 33 olguda cerrahiyle ve 19 olguda klinik takiple konuldu. Bulgular: BT ile olguların 27'sinde doğru pozitif, üçünde yalancı pozitif, yirmisinde doğru negatif, ikisinde yalancı negatif sonuç elde edildi. Bu değerlere göre tüm olgularda BT'nin sensitivitesi %93.1, spesifısitesi %86.9, pozitif prediktif değer (PPD) %90, negatif prediktif değer (NPD) %90.1 olarak bulundu. US ile 51 olgunun 24'ünde doğru pozitif, 2'sinde yalancı pozitif, 21'inde doğru negatif, 4'ünde yalancı negatif sonuç saptandı. Bu verilere göre tüm olgularda US'nin sensitivitesi %85, spesifısitesi %91,PPD %93, NPD %84 olarak bulundu. Sonuç: US ve BT'nin sensitivite ve spesifisite değerleri birbirine oldukça yakındır. Ancak kolay uygulanabilir olması, ucuz olması ve iyonizan radyasyon içermemesi sebebiyle ilk görüntüleme modalitesi US olmalıdır. US bulguları ile klinik bulguların uyumsuzluğu halinde ince kesit kontrastsız BT tetkiki tercih edilmelidir.
