The relationship between laparoscopic upper abdominal surgery complications and pulmonary tests

Abstract

Aim: The pulmonary complications after upper abdominal surgery are the main causes of the prolonged stay in the hospital. On the other hand there is no specific test to predict the postoperative pulmonary complications (PPC). Methods: The aim of the present study was to determine the risk factors for development of PPC after laparoscopic cholecystectomy (LC). Fourty patients who underwent LC were enrolled to the study. The medical data of perioperative period were recorded. Pulmonary function tests, chest radiography and physical examination were repeated in preoperative and postoperative period including firsth, 7th, and 15th days after the operation. Results: According to the perioperative period, on the first day of operation there were significant FEV1 and FVC decline (p=0.001 and p=0.001, respectively). FEV1 and FVC values returned to normal between 7th and 30th days after the operation. The arterial blood gases were also decreased significantly after the operation compared to preoperative arterial blood gases. We showed that the PPC were seen in the first week of laparoscopic cholecystectomy. Conclusions: In Conclusion the patients should be evaluated carefully before the operation to reduce the risks and complications in the perioperative period.

Amaç: Üst batın cerrahisi sonrası gelişen pulmoner komplikasyonlar hastanede kalış süresini uzatan en sık nedenlerdendir. Ancak postoperatif pulmoner komplikasyonları (PPK) önceden belirleyen spesifik bir test henüz bulunmamaktadır. Metod: Bu çalışmada amaç laparoskopik kolesistektomi sonrası hastaya ve cerrahi işlemlere ait risk faktörleri ile PPK ilişkisini araştırmaktadır. Çalışmaya laparoskopik kolesistektomi uygulanan 40 hasta alındı. Hastaların operasyon öncesi ve sonrası kayıtları ileriye dönük incelendi. Tüm hastaların operasyon öncesi ve operasyonun 1., 7. ve 30. günlerinde solunum fonksiyon testleri (SFT), fizik muayene ve akciğer grafisi bulguları kaydedildi. Ayrıca operasyon öncesi ve operasyonun 1. gününde arteriyel kan gazları (AKG) tekrarlandı. Bulgular: Preoperatif döneme ait SFT bulgularının postoperatif 1. gün, 7. gün ve 30. günlere ait FEV1, FEV1%, FVC ve FVC% karşılaştırıldığında, preoperatif dönem FEV1, FVC değerinin postoperatif 1. gün FEV1 ve FVC’e oranla azalması belirgin olarak saptandı (p=0.001 ve p=0.001, sırasıyla). Aynı zamanda FEV1 ve FVC değerleri postoperatif 7. gün ile 30. gün değerlerinin preoperative dönem değerlerine doğru artış gösterdiği tespit edildi. Preoperatif ve postoperatif 1 günlerde alınan AKG’ları karşılaştırıldığında, ortalama PO2 ve saturasyonların postoperatif 1 günde düştüğü izlendi (p=0.001). PPK’ ların özellikle ilk hafta içinde daha fazla görülmesi nedeniyle, bu dönemde hastalar yakın takip edilmelidir. Bu durum operasyon sonrası ilk haftada izlenen SFT ve AKG değişiklikleri, akciğer grafisi ve fizik muayene bulguları ile desteklenmektedir. Sonuç: Sonuç olarak tüm hastalar operasyon öncesi iyi değerlendirilmeli ve riskler belirlenmelidir. Bu riskleri belirlemede SFT hem tanıda hem de uygun perioperatif tedaviyi belirlemede yardımcıdır.
