Physicians' drug preferences and polypharmacy presence in inpatients of university hospital

Abstract

ObjectiveThe purpose of this study; to analyze the active ingredients used and to detect the presence of polypharmacy, to determine the effect on duration of hospitalization and age of inpatients in a 3rd place health center.Material-MethodThis cross sectional descriptive study was performed in a university hospital with 600 beds capacity, to cover date of 1-31 December 2013 from 2162 total patient files. The data used was obtained from the hospital information technology departments with taking necessary permission. In the study, used amounts of active ingredient according to age, the most commonly used active ingredients and the services used these active ingredients, the services most frequently used agents, patients' length of stay, and relationship with polypharmacy were evaluated. Relationship with the length of stays and age distribution of patients with polypharmacy were analyzed by Mann-Whitney U test.ResultsOne-month period 59.301 active agents were used in 2162 a total of patients, and polypharmacy were found in 1611 patients (74.52 %). It was detected patients with polypharmacy are in advanced age and polypharmacy increased the length of hospital stay and used number of active ingredient significantly (p<0.001). Lansoprazole (2729 units, 4.60 %), Enoxaparin sodium (2478 units, 4.17%) and Paracetamol (1826 units, 3.07%) are the most commonly three active ingredients used in hospital services respectively. The services which used the maximum number of active ingredients has been identified as General surgery (7002 units, 11.8 %) and gynecology and obstetrics (5103 units, 8.6 %). DiscussionIt was determined that drug treatment had significant effects on age, length of stay and the presence of polypharmacy in inpatients who stayed different services of university hospital.

Özet

Amaç Çalışmamızın amacı bir 3. basamak sağlık merkezi olan üniversite hastanesinde, kullanılan etken maddelerin analizini yapmak, polifarmasi varlığını saptamak ve polifarmasinin hastaların yaşı ve hastanede kalış süresine olan etkilerini araştırmaktır. Materyal -MetotKesitsel tanımlayıcı tipteki bu çalışma, 600 yatak kapasiteli bir üniversite hastanesinde, 1-31 Aralık 2013 tarihleri arasındaki toplam 2162 hasta bilgileri kullanılarak yapılmıştır. Kullanılan veriler, hastane bilgi işlem dairesinden alınarak değerlendirilmiştir. Çalışma için gerekli izinler alınmıştır Çalışma kapsamında yaşlara göre kullanılan etken madde miktarları, en sık kullanılan etken maddeler ve kullanıldığı servisler belirlenmiştir. Hastaların yatış süreleri ve yaş dağılımının polifarmasi ile olan ilişkisi Mann-Whitney U testi ile değerlendirilmiştir.BulgularBir aylık periyod içerisinde 2162 hastada toplam 59301 etken madde kullanılmış ve bu hastaların 1611'inde (%74,52) polifarmasi saptanmıştır. Polifarmasi varlığı saptanan hastaların daha ileri yaşlarda olduğu, hastanede kalış süresinin ve etken madde kullanım sayısının anlamlı olarak daha fazla olduğu saptanmıştır (p<0,001). Hastane servislerinde en sık kullanılan ilk üç etken madde sırasıyla Lansoprazol (2729 adet, %4,60), Enoksaparin Sodyum (2478 adet, %4,17) ve Parasetamol'dür (1826 adet, %3,07). Genel cerrahi (7002 adet, %11,8) ile kadın hastalıkları ve doğum (5103 adet, %8,6) servisleri en fazla etken maddenin kullanıldığı bölümler olarak belirlenmiştir.Sonuç Üniversite hastanesinin farklı servislerinde yatan hastalara uygulanan ilaç tedavilerinin yaş, kalış süresi, polifarmasi varlığına önemli etkileri olduğu belirlenmiştir. Elde edilen verilerin sonraki ışık tutacağı kanaatindeyiz.
