Physiopathological value of interleukin-6 in the patients with primary intracerebral haemorrhage

Abstract

There are many reports on the role of proinflammatory cytokines (tumor necrosis factor alpha, interleukin Iß, interleukin-6) on brain injury due to cerebral ischemia. However, the role of inflammatory process on the brain injury due to primary intracerebral hemorrhage (PIH) is not properly explained. With this aim, we analysed interleukin-6 (IL-6) levels of 44 patients with PIH on the first day and 4th day and compared them to the IL-6 levels of 30 healthy subjects. On the first day; mean IL-6 level of the patient group was 250.51 ± 132.39 pg/ml, and mean IL-6 level of the control group was 85.43 ± 74.01 pg/ml. There was statistical difference between the two groups (p < 0.001 ). On the 4th day; mean IL-6 levels of the patient group was 160.63 ± 110.14 pg/ml, and mean IL-6 levels of the control group was 85.43 ± 74.01 pg/ml. There was statistical difference between the two groups (p<0.001). When we compared the patients' IL-6 levels on the first day to the levels on the 4th day, again there was statistical difference (p < 0.001). According to these statistical datas; on the first day of PIH, IL-6 level increases and then starts to fall down, but on the 4th day it is still comparably high. In conclusion, the study showed that there is meaningful increase on IL-6 levels of the patients with PIH, in the earliest phase of the event. This result may support the hypothesis of the role of proinflammatory cytokines on the brain injury and for developing brain edema around the periphery of the intracerebral hemorrhage but precise conclusion more detailed studies are needed.

Serebral iskemiye bağlı beyin hasarı oluşumunda, proinflamatuvar sitokinlerin (tümör nekrosis faktör alfa, interlökin-lß, interlökin-6) rolü üzerine birçok çalışma yapılmıştır. Ancak insanlarda primer intraserebral kanamada (PİK) beyin hasarı oluşumunda inflamatuvar sürecin rolü henüz aydınlatılmamıştır. Bu amaçla PlK'lı 44 hastada, kanamanın 1 ve 4. günlerinde lL-6 düzeylerini ölçtük. Bu düzeyleri sağlıklı 30 bireyin 1L-6 düzeyleri ile kıyasladık. 1. gün hasta grubunda IL-6 düzeyi ortalama; 250.51 ± 132.39 pg/ml kontrol grubunda ise ortalama; 85.43 ± 74.01 pg/ml idi. iki grup arasında istatistiksel olarak anlamlı bir fark vardı (p < 0.001). 4. gün hasta grubunun lL-6 düzeyi ortalama 160.63 ± 110.14 pg/ ml, kontrol grubunun ise ortalama; 85.43 ± 74.01 pg/ml idi. Yine iki grup arasında istatistiksel olarak anlamlı bir fark vardı (p<0.001). Yalnızca hasta grubunun 1. ve 4. gün lL-6 düzeylerini karşılaştırdığımızda ise bu iki gün arasında da istatistiksel olarak anlamlı bir fark vardı (p<0.001). Bu istatistiksel verilere göre PlK'larda lL-6 1. günde artmakta ve daha sonra giderek düşse de 4. günün sonunda normal sınırlarına dönmemektedir. Sonuç olarak bu çalışma, PİK'larda lL-6'nın anlamlı oranda arttığını göstermiştir. Bu sonuç, kanama çevresinde oluşan beyin hasarında, beyin ödeminde proinflamatuvar sitokinlerin rolü olabileceği hipotezini destekleyebilir ancak bu konuda daha fazla çalışmaya gerek vardır.
