Demographic features and risk factors of ischemic stroke subtypes in a hospital-based study

Abstract
The classification of ischemic cerebrovascular diseases (1CVD) is important in respect to treatment, prognosis and secondary prevention of acute ischemia. Determining the risk factors is also important for primary prevention and also may provide significant contributions to public health and national economic issues.. We studied 850 patients who had been hospitalised in Haydarpaşa Numune Education and Research Hospital 2nd Neurology Clinics with a diagnosis of 1CVD during September 1998 to July 2002. We excluded cases died acutely and cases with sinus thrombosis and whom we could not complete the investigation and follow up procedure. Remaining 439 patients were classified due to 1991 Bamford and 1993 TOAST studies. We determined the probable and certain risk factors. 193 (%44) of 439 patients were men, 246 (%56) were women. Age of the patients were between 25 and 90 years with a mean of 62.93±12.12. Among risk factors that could be assigned hypertension was the leading one with a prevalance of %65 and this was found highly significant among others. Lesions located in anterior circulation were found to be more prevalent than posterior circulation lesion. Strokes due to small vessel disease were higher than other causes.

Iskemik serebrovasküler hastalıklarda (İSVH) sınıflandırma, akut iskeminin tedavisi, prognozu ve ikincil korumada oldukça önemlidir. Risk faktörlerinin belirlenmesi ise birincil korunmaya yönelik olup toplum sağlığı ve ülke ekonomisine önemli katkılar sağlamaktadır. Biz, Haydarpaşa Numune Eğitim ve Araştırma Hastanesi 2. Nöroloji Kliniğinde Eylül 1998-Temmuz 2002 tarihleri arasında tSVH tanısıyla yatırmış olduğumuz 850 hastayı inceledik. Akut dönemde exitus olanları, sinüs trombozu tanısı alanları, herhangi bir nedenle araştırma planım tamamlayamayanları (ekonomik nedenler, tetkiki kabul etmeme, takibe gelmeme) dışladık. 850 hastadan 439 tanesinin dökümünü hazırladık. Hastalarımızı 1991 Bamford ve 1993 TOAST çalışmalarını referans alarak sınıflandırdık. Kesin ve muhtemel risk faktörlerini belirledik. 439 İSVH' nın 193'ü erkek (%44), 246' sı (%56) kadındır. Olguların yaşlan 25 ile 90 arasında değişmekte olup yaş ortalaması 62.93±12.12 dir. Hastalarda tespit edilen risk faktörlerinden hipertansiyon (%65.4) birinci sırayı alarak diğerlerinden anlamlı olarak yüksek bulunmuştur(p<0.01). Lokalizasyona göre değerlendirmede ön sistem tutulumu arka sistem tutulumundan daha yüksek, küçük damar hastalığına bağlı inmeler de diğer nedenlere göre daha yüksek oranda bulundu.
