The incidence of thyroid carcinoma in patients who were managed based on dominant nodule in benign nodular thyroid disease

Abstract

Objective: Frequency of the incidental thyroid carcinoma (ITC) is variable in patients operated on for benign nodular thyroid disease (BNTD). The aim of this study is to determine the incidence of incidental carcinoma or microcarcinoma among patients with BNTD, to investigate the malignancy rate in dominant or non dominant nodule and discuss our operation choice in patients having BNTD. Material and methods: The records of 643 patients operated for BNTD were reviewed retrospectively. Initially the patients data were divided into 5 groups according to the dominant nodule size that was identified and measured by US and finally patients were classified in two groups as benign disease (BD) and ITC according to the final pathology results. Demographic and clinical features were compared in two groups. In the cases with ITC, the tumor’s location in dominant or non dominant nodule and the effect of the nodule diameter on malignancy in tumors originating from dominant nodule were evaluated. Results: ITC rate was found as 30/643 (4.7%) in our series and false negativity rate of FNAC in dominant nodules was found as 16/643 (2.5%). Number of tumor cases in dominant nodule was similar with the non dominant nodules as 16 and 14, respectively. The most preferred surgical procedure was bilateral subtotal thyroidectomy (BST) which was applied to 283 patients, while total thyroidectomy (TT) and near total thyroidectomy (NTT) were applied 288 patients in total. Conclusion: According to these results dominant nodule may be misleading in patient management and choice of operation.

Özet

Amaç: Benign nodüler tiroid hastalıkları (BNTH) için ameliyat edilen hastalarda insidental tiroid karsinom (İTK) sıklığı değişkendir. Bu çalışmanın amacı BNTH nedeni ile ameliyat edilen hastalardaki İTK sıklığını belirlemek, dominant ve non dominant nodüllerdeki malignite oranlarını araştırmak ve bu hastalardaki operasyon seçimlerimizi tartışmaktır Gereç ve Yöntem: BTNH nedeni ile ameliyat edilen 643 hastanın kayıtları retrospektif olarak incelendi. Başlangıçta hastalar ultrasonografi (US) ile ölçülen dominant nodül çaplarına göre 5 gruba, son tanılarına göre de benign hastalık (BH) ve İTK olarak 2 ana gruba ayrıldı. İki gruptaki hastalar demografik ve klinik özelliklerine göre karşılaştırıldılar. İTK saptanan hastalarda tümörün dominant nodülde mi, non dominant nodülde mi bulunduğu ve eğer dominant nodülde ise nodül çapı araştırıldı. Bulgular: Bizim serimizde İTK oranı %4.7 (30/643) olarak tespit edildi. Dominant nodülden yapılan ince iğne aspirasyon biyopsilerindeki (İİAB) yanlış negatişik oranı %2.5 (16/643) olarak bulundu. İTK tespit edilen vakaların 16’sında tümör dominant nodülde 14’ünde ise non-dominant nodülde bulundu. En çok uygulanan cerrahi prosedür 283 hasta ile bilateral subtotal tiroidektomi (BST) iken, total tiroidektomi (TT) ve near total tiroidektomi (NTT) toplamda 288 hastaya uygulanmış olarak tespit edildi. Sonuç: Bu sonuçlara göre, BNTH’da ameliyat kararı almanın ve alınan karar sonrası ameliyat tipinin seçiminin sadece dominant nodüle göre planlanmasının yanıltıcı olabileceğini düşünmekteyiz.
