Expression of depression in major depressive disorder patients in Şanlıurfa

Abstract
Objective: The expression of depression was investigated by determining the application complaints of patients who were diagnosed as major depressive disorder. Methods: The charts of 100 consecutive patients who were diagnosed as major depressive disorder in the psychiatric outpatient clinic were revietved in order to determine the application complaints. Results: Seventy-four types of expressions were determined. Thirty-one of the expressions were related with the emotional domain, while 30 were related with the somatic domain and 13 were related with the cognitive domain, Emotional complaints were present in %96 of the patients, while somatic complaints were present in %87 and cognitive complaints were present in %44. The most common emotional complaints were distress (%55), nervousness (%38) and anhedonia (%25) respectively. The most common somatic complaints were insomnia (%52), headache (%21) and fatigue (%21). The most common cognitive complaints were feeling overloaded (%15), suicidal thoughts (%13) and worrying about everything (%11). The reporting of emotional, somatic and cognitive expressions was not related with sex, age, employment status and educational status (p>0.05). There were significantly more cognitive complaints in single patients when compared with the married patients (p<0.05), while emotional and somatic complaints did not differ between the two groups (p>0,05). Conclusion; The most common complaint in depressive patients was "distress" which belongs to the emotional domain. People who report distress, insomnia and nervousness as a complaint should be evaluated for depression.

Amaç: Majör depresyon tanısı konulan kişilerin başvuru şikayetleri saptanarak, depresyonlarını ifade etme biçimlerinin araştırılması amaçlanmıştır, yöntem: Psikiyatri polikliniğine başvuran ve klinik olarak majör depresyon tanısı konulan ardışık 100 kişinin dosyaları taranarak başvuru şikayetleri saptandı. Bulgular: Çalışmamızda yetmiş dört farklı ifade sayısı saptandı. İfadelerin 317 duygusal, 30'u bedensel, 13'ü de bilişsel alan ile ilgilidir. En sık yakınmalar duygusal alanla ilgilidir (%96). ikinci sıklıkta bedensel ifadeler (%87), en az sıklıkta bilişsel alanla ilgili ifadeler (%44) yer almaktadır. En sık belirtilen duygusal ifadeler sırasıyla sıkıntı (%55), sinirlilik (%38), isteksizliktir (%25). Belirtilen 30 bedensel ifadeden en sık üç ifade sırasıyla uykusuzluk (%52), başağrısı (%21), halsizliktir (%21). Hastaların belirttiği 13 bilişsel ifadeden en sık üç tanesi ise, işlerin gözünde büyümesi (%15), intihar düşüncesi (%13), her şeyi kafaya takma (%11) biçimindedir, öuygusal, bedensel ve bilişsel ifadeler in bildirilme sıklığı cinsiyete, yaş gruplarına, çalışma durumuna ve öğrenim durumuna göre anlamlı farklılık göstermemektedir (p>0.05). Bilişsel ifadeler bekar olanlarda evli olanlara göre anlamlı olarak fazla bulunurken (p 0.05). Sonuç: Depresyonu olan kişiler en sık olarak "sıkıntı" ifadesini kullanarak duygusal alanla ilgili yakınma bildirmektedirler. Sıkıntı, uykusuzluk ve sinirlilik şikayetleri bildiren kişilerin depresyon açısından sorgulanması gereklidir.
