Is it possible to reduce the incidence of central venous catheter malposition?

Abstract

The aim of this study was to investigate the incidence of catheter malpositions in our hospital, their locations and complications. The files of 911 adult patients who had been operated on by the department of cardiovascular surgery with CVC between 2003 and 2004 were evaluated retrospectively. Catheter locations were determined and recorded postoperatively with chest radiographies in the ICU. It was found that central venous catheters had been placed on the right in 95.3% and on the left in 4.7% of the patients. The internal jugular vein (IJV) was selected for catheterization in 95.1%, the subclavian vein (SCV) in 4.6% and the external jugular vein (EJV) in 0.3%. A malposition incidence of 20.5% for the catheters inserted on the right and 11.6% for the catheters inserted on the left were reported with a total malposition rate of 20.1%. The malposition rate was 20.6% for right and 10% for left IJV, 16.7% for right and 16.7% for left SCV. Malposition was detected in 1 out of 3 catheters that were placed at EJV. The most common malpositions were reported for the right atrial position (18.3%). Malposition of CVC could be overlooked in the absence of any monitorization technique during the application of CVC. In particular, use of a short catheter is important in preventing an intracardiac placement while catheter malposition should be eliminated with scopy or chest radiographies. If the blood cannot be seen from one of the catheter lumens, this could lead to a the suspicion of malposition.

Santral venöz kateter (SVK) malpozisyonları kateterin yerleşim yerine bağlı olarak ciddi komplikasyonlara yol açabilmektedir. Çalışmamızın amacı; hastanemizde takılan kateterlerin malpozisyon sıklığını, bunun girişim yeri ile ilişkisini ve komplikasyonlarını araştırmaktır. 2002-2004 yılları arasında kalp cerrahisi tarafından öpere edilen ve SVK uygulanan 911 erişkin hasta dosyası retrospektif olarak değerlendirildi. Kateter izasy on için seçilen damarlar, girişim sırasında ve sonrasında görülen komplikasyonlar, ameliyat sonrası yoğun bakımda çekilen akciğer grafileri ile belirlenen kateter pozisyonları kaydedildi. SVK'lerin % 95.3'ünün sağdan, % 4.7'sinin soldan takılmış olduğu ve % 95.1'inde internal juguler ven, % 4.6'sında subklav-yen ven, % 0.3'ünde eksternal juguler venin tercih edildiği gözlendi. Sağdan yerleştirilen kateterlerin % 20.5'inde soldan yerleştirilen kateterlerin ise % 11.6'sında olmak üzere toplamda kateterlerin % 20.1'inde malpozisyon saptandı, internal juguler vende sağda % 20.6, solda % 10, subklavyen vende sağda % 16.7, solda % 16.7 oranında, eksternal juguler vende ise takılmış olan 3 kateterden 1 tanesinde malpozisyon bulundu. En sık görülen malpozisyon sağ atrium malpozisyonuydu (% 18.3). Kayıtlarda malpozisyon dışında SVK yerleştirilmesine ait herhangi bir komplikasyona rastlanmadı. Santral venöz kateter uygulaması sırasında herhangi bir görüntüleme veya monitor izasy on yöntemi kullanılmadığında malpozisyon gözden kaçabilir. Özellikle intrakardiyak yerleşimin önlenmesinde kateter boyunun kısa tutulması önemli iken, kateter lümenlerinden birisinden kan gelmediğinde malpozisyondan şüphelenilip skopi veya akciğer filmi ile kateter malpozisyonunun ekarte edilmesi gerektiğini düşünüyoruz.
