The risk factors affecting thrombosis formation related to internal jugular vein catheterization in open heart surgery

Abstract
The aim of this study was to investigate the incidence of catheter related thrombus formation and to determine precipitating factors following internal jugular vein catheterization in patients undergoing open heart surgery. Ninety patients scheduled for open-heart surgery participated in the study. Immediately after endotracheal intubation, one a triplelumen central venous catheter), two (a triple-lumen central venous catheter and an introducer), or three (a triple-lumen central venous catheter, an introducer, and a pulmonary artery catheter) catheters were inserted through the right internal jugular vein, based on the clinical condition of the patient and the planned surgical intervention. Internal jugular vein thrombus formation was studied postoperatively within 48 hours and after 10 and 30 days following the removal of the catheter using Doppler ultrasonography Logistic regression analysis was used to evaluate the effect of risk factors for thrombus formation such as repeated attempts of venous puncture, hematoma formation, number of catheters inserted, catheter placement exceeding 72 hours, and postoperative administration of acetyl salycilic acid or warfarin. Statistical analysis of findings from 82 patients revealed a 34.1 % overall incidence of thrombus formation at first the Doppler ultrasonographic evaluation. Internal jugular venous thrombosis was shown in 20% of patients with a single triple-lumen central venous catheter, in 26% of patients with a triple-lumen catheter and an introducer, and in 50% of patients with double catheters and a pulmonary artery catheter. The incidence of thrombus formation was shown to be 20.5% when the duration of catheter placement was less than 72 hours, whereas it was 46.5% when the duration exceeded 72 hours. Pulmonary embolism and catheter sepsis are major risks associated with catheter related thrombi. A great majority of internal jugular venous thrombi following open heart surgery were observed to resolve 10 days after the operation. The number of catheters inserted through the same vessel and the duration of catheter placement were two factors shown to significantly increase thrombus formation.

Çalışmanın amacı açık kalp cerrahisinde internal juguler venin kateterizasyonuna bağlı postoperatif dönemde oluşan trombüs insidansını araştırmak ve etki eden faktörleri belirlemektir. Açık kalp ameliyatı yapılması planlanan 90 erişkin hastaya endotrakeal entübasyondan hemen sonra, hastanın klinik durumuna ve yapılacak ameliyata göre sağ internal juguler vene (İJV) bir (üç yollu santral venöz kateter), iki (üç yollu kateter ve introduser) veya üç (üç yollu kateter, introduser ve pulmoner arter kateteri) adet kateter yerleştirildi. Postoperatif dönemde kateterler çekildikten sonra 48 saat içinde, 10 ve 30 gün sonra internal juguler ven Doppler ultrasonografileri incelendi. Kateterizasyon sırasında yapılan ponksiyon sayısı, hematom gelişmiş olması, kateter sayısı, kateterlerin 72 saatten uzun kalması, postoperatif asetil salisilik asit veya warfarin kullanımı gibi kateterizasyona bağlı trombüs riskini etkileyebilecek faktörlere lojistik regresyon analizi uygulandı. istatistiksel eğerlendirmeye 82 hasta alındı. ilk Doppler ultrasonografide trombüs insidansı % 34.1 olarak bulundu. Bu çalışmada açık kalp cerrahisinde iJV'nin kateterizasyonuna bağlı erken dönemde saptanan trombüslerin büyük çoğunluğunun operasyondan 10 gün sonra kaybolduğu gözlemlendi. Kateter sayısın artmasının ve kateterin kalış süresinin 72 saati geçmesinin trombüs riskini ve trombüsün büyüklüğünü artırdığı gösterildi.
