NEGATIVE PRESSURE PULMONARY EDEMA DEVELOPED AFTER GENERAL ANESTHESIA

Abstract
Introduction: Negative pressure pulmonary edema (NPPE) is a rare complication of general anesthesia (GA); which can be seen during extubation and in the early post-operative period. It may require intensive care and mechanical ventilation, and can be fatal. In this case report, we aimed to emphasize the importance of considering NPPE in hypoxia which may be seen following extubation in patients receiving GA. Case report: The present patient case was a 67-year-old man, weighting 88 kg and measuring 175 cm in height. He was an ASA class II, hypertensive patient who was scheduled for an inguinal hernia repair under GA. The patient developed respiratory distress and agitation after extubation following an uneventful intubation and surgery. Peripheral oxygen saturation decreased to 70%. He was re-intubated. Echocardiography showed no evidence of a cardiac problem. The patient was diagnosed with NPPE based on the chest radiographic observation which showed an extensive infiltration consistent with pulmonary edema in both lungs. The patient who was treated in the intensive care unit for three days was transferred to the ward. On the fourth day he was discharged home. Discussion: This is an intriguing case report since this patient was at an advanced age and experienced dramatic hemodynamic changes. NPPE should be brought to mind in patients who develop hypoxia following extubation, as this patient, and rapid intervention should be provided.

Giriş: Negatif basınç akciğer ödemi (NBAÖ), ekstübasyon sırasında ve postoperatif erken dönemde görülebilen, yoğun bakım ve mekanik ventilasyon gerektirebilen, nadir fakat fatal olabilecek bir genel anestezi (GA) komplikasyonudur. Bu olgu sunumunda GAuygulanan hastalarda ekstübasyon sonrası görülen hipokside NBAÖ’nün akla getirilmesinin önemini vurgulamayı amaçladık. Olgu sunumu: Hastamız; 67 yaşında, 88 kg, 175 cm boyunda, ASA II, hipertansif erkek hasta idi. GA ile inguinal herni onarımı yapılacaktı. Sorunsuz entübasyon ve cerrahiyi takiben ekstübasyon sonrası hastada solunum sıkıntısı ve ajitasyon gelişti. Periferik oksijen saturasyonu %70’e kadar düştü. Hasta yeniden entübe edildi. Hastaya yapılan ekokardiyografide kardiyak sorun saptanmadı. Çekilen akciğer grafisinde her iki akciğerde pulmoner ödem ile uyumlu yaygın infiltratif görünüm saptanınca NBAÖ tanısı konuldu. Yoğun bakımda takip edilen hasta 3. gün servise çıkarıldı, 4. gün evine taburcu edildi. Tartışma: Olgumuz literatüre bakıldığında ileri yaşı ve çarpıcı hemodinamik değişikleri ile ilgi çekicidir. Hastamızda olduğu gibi ekstübasyon sonrası görülen hipokside NBAÖ akla getirilmeli ve hızlı müdahale uygulanmalıdır.
