A comparison of hemodynamic and respiratory functions between prone and knee-chest position during lumber disc surgery under spinal anesthesia

Abstract
Objective: Lumbar disc surgery may be carried out under regional or general anesthesia using different positions. The aim of this study was to compare the hemodynamic and respiratory functions between prone and knee-chest position used in patients undergoing lumbar disc surgery under spinal anesthesia. Methods: Thirty-six patients aged 18-60 and undergoing lumbar disc surgery were prospectively randomized into 2 groups after spinal anesthesia. During surgery, the prone position was used in Group I (n-18), while the knee-chest position was used in Group II (n-18). Using portable spirometry, respiratory function tests were recorded preoperatively, after spinal anesthesia, 15 minutes after taking the prone or knee-chest position and at the end of surgery. Peripheral oxygen saturation, heart rate and blood pressure were also measured during the same periods. Results: The patients' demographics and surgical features were similar. There were no significant differences between the groups regarding hemodynamic parameters and forced expiratory volume at one second (FEV1), forced vital capacity (FVC) measurements (p>0.05). In Group I, a decline in FVC measurements indicating restriction was observed after spinal anesthesia and 15 minutes after knee-chest position (p=0.006 and p=0.01 respectively). Peak expiratory flow (PEF) measurements significantly decreased after the knee-chest position when compared with the prone position (p=0.035). Conclusion: The position used in lumbar disc surgery under spinal anesthesia does not affect the hemodynamic parameters. However, when compared with the patients given the prone position, restrictive alterations were more significant in the respiratory functions öf patients in the knee-chest position.

Amaç: Lomber disk cerrahisi bölgesel veya genel anestezi altında farklı pozisyonlarda gerçekleştirilebilmektedir. Bu çalışmanın amacı, spinal anestezi altında lomber disk cerrahisi yapılan hastalarda, pron ve diz-dirsek pozisyonlarının hemodinami ve solunum fonksiyonları üzerine olan etkilerini karşılaştırmaktır. Yöntem: Lomber disk cerrahisi geçirecek yaşları 18-60 arası 36 hasta prospektif randomize olarak spinal anestezi sonrası 2 gruba ayrıldı. Cerrahi için Grup 1'de (n=18) diz-dirsek, Grup 2'de (n-18) ise düz pron pozisyon kullanıldı. Taşınabilir spirometre ile preoperatif spinal anestezi sonrası, diz-dirsek veya düz pron pozisyon sonrası' 15. dakikada ve cerrahi sonunda solunum fonksiyon testleri (SFT) yapıldı. Aynı dönemlerde periferik oksijen satürasyonu, kalp atım hızı ve kan basıncı değerleri kaydedildi. Bulgular: Hastaların demografik ve cerrahi özellikleri benzerdi. Hemodinamik değişiklikler ve birinci saniyedeki zorlu ekspiratuvar volüm (FEV1), zorlu vital kapasite (FVC) değerleri için gruplar arasında anlamlı fark bulunmadı (p>0.05). Grup V de spinal anestezi ve diz dirsek pozisyonu sonrası 15 .dakikada restriksiyon ile uyumlu olarak FVC de düşme saptandı (p=0.006 ve p=0.01, sırasıyla). Diz dirsek pozisyonu verildikten sonra Grup Vdeki maksimum ekspiratuvar akım (PEF) değeri Grup 2'de düz pron verildikten sonraki değere göre düşük bulundu (p=0.035). Sonuç: Spinal anestezi uygulanan lomber disk ameliyatlarında verilen pozisyonların hemodinamik açıdan olumsuz etkisinin bulunmadığı, diz-dirsek pozisyonunun pron pozisyona göre solunum fonksiyonlarında daha belirgin restriktif değişikliğe neden olduğu saptanmıştır.
