Subcutaneous emphysema and hypercarbia during laparoscopic total extraperitoneal inguinal hernia repair

Abstract
Laparoscopic total extraperitoneal (TEP) requires CO2 insufflation to the preperitoneal region. Pneumothorax, pneumomediastinum and subcutaneous emphysema are unexpected and rare complications in laparoscopic TEP inguinal hernia repair. A 32- yr-old, 70 kg, ASA I, male patient scheduled for bilateral inguinal hernia repair is presented. His preoperative history, physical examination and labaratory values were unremarkable. After induction, anesthesia was maintained by 50-50% O2-NO2 and 4-6% desflurane inhalation. During TEP hernia repair method ETCO2 increased to 36 mmHg to 48 mmHg. Subcutaneous emphysema was palpated in the upper wall of the thorax and, especially in the left side of the neck. The laparoscopy procedure was not terminated due to the stable haemodynamic status of the patient’s heart rate, blood pressure and peripheral oxygen saturation. CO2 insufflation pressure was decreased 50%. NO2 was stopped and anesthesia was maintained by 100% oxygen and desflurane inhalation. The patient was sent to his bed after one hour follow up in the postanesthesia care unit when regression was seen in the emphysema. There was no crepitation palpated in neck and upper wall or thorax in his examination after postoperative 6th hour. The basic principle in preventing complications related to laparoscopic surgery (pneumothorax, pneumomediastinum, subcutaneous emphysema, hypercarbia); to take the operation times as shorter as possible and decreasing the insufflation pressures to minimum. The moment the hypercarbia and subcutaneous emphysema was realised; in accordance with literature; the N20 was stopped, the CO2 insufflation pressure was reduced 50%, the patient was hyperventilated with %100 O2, and the operation was ended in a short time after establishing a cooperation with surgeons. The patients subcutaneos emphysema was receded in postoperative care unit and no complication and pathology was developed and he was discharged at postoperative 24th hour.

Alıştığımız laparoskopik girişimlerden farklı olarak, laparoskopik ekstraperitoneal işlemlerde CO2 insüflasyonu preperitoneal bölgeye yapılır. Pnömotoraks, pnömomediastenium ve ciltaltı amfizemi laparoskopik total ekstraperitoneal (TEP) inguinal herni tamirinde beklenmeyen ve nadir komplikasyonlardır. 32 yaşında, 70 kg, ASA I, erkek hasta bilateral kasık fıtığı tamiri için hazırlandı. Preoperatif anamnez, fizik muayene ve laboratuar tetkiklerinde özellik yoktu. İndüksiyon sonrası anestezi idamesi; %50-50 N2O-O2 ve % 4-6 desfluran ile sağlandı. Laparoskopik TEP herni tamiri yöntemi ile inguinal herni tamiri sırasında ETCO2 36 mmHg’dan 48 mmHg’ya yükseldi. Toraks üst duvarı ve özellikle boynun sol tarafında ciltaltı amfizemi palpe edildi. Hastanın kalp atım hızı, kan basıncı, periferik oksijen saturasyonu bozulmadığından laparoskopi işlemi sonlandırılmadı, CO2 insüflasyon basıncı %50 azaltıldı. N2O kesildi, anestezi idamesi %100 O2-desfluran ile sağlandı. Herni tamiri tamamlandıktan sonra derlenme odasında bir saat takip edilen hastada amfizemin gerilediği görüldüğünde servisine gönderildi. Postoperatif 6. saatte yapılan muayenede boyun ve toraks üst duvarında krepitasyon palpe edilmedi. Laparoskopik cerrahi ile ilişkili komplikasyonların (pnömotoraks, pnömomediastenium, ciltaltı amfizemi, hiperkarbi) önlenmesinde temel ilke; operasyon sürelerinin mümkün olduğunca kısa tutulması ve insüflasyon basınçlarının en aza indirilmesidir. Hastamızda gelişen hiperkarbi ve cilt altı amfizemi fark edildiğinde; literatürle uyumlu olarak azot protoksit kesildi, CO2 insüflasyon basıncı %50 azaltıldı, hasta %100 O2 ile hiperventile edildi, cerrahi ekip ile kooperasyon sağlanarak operasyon kısa sürede sonlandırıldı. Derlenme odasında cilt altı amfizemi gerileyen hasta, ek komplikasyon ve patoloji gelişmemesi üzerine 24. saatte taburcu edildi.
