DECREASE OF SEDATION AND GENERAL ANESTHESIA RATE WITH DISTRACTION TECHNIQUES IN CHILDREN DURING MAGNETIC RESONANCE IMAGING

Abstract

Obj ective: MRI in children is hard to complete with high resolution due to noise, long duration of procedure and the need to stand still. The aim of the study is to investigate the ratio of anesthesia or sedation requirement in pediatric patients f ollowing one of the psychological distraction techniques or being close to parents or with babies, breast f eeding during the MRI procedure. Metods: In this retrospective study we retrospectively analyzed the data of 60 patients, 1 month to 4 years, who receive MRI. All children were accepted into the machine with their parents. In order to reduce noise the external ear canals were plugged with cotton. Toys or candies were used to distract the anxiety of children to keep them stay still. Children under 12 months were breast f ed and put to sleep bef ore introducing to the MRI. If all the above were unsuccessf ul, only then patients were sedatized or given anesthesia. Results: Five patients under 12 months were imaged during breast f eeding and sleeping. In patients older than 12 months, 36 of them (%65.5) completed the imaging without receiving anesthesia. Seven of the patients (%12.7) received sedation and twelve (%21.8) received general anesthesia. Sixty-f ive point f ive percent (65.5%) of patients did not receive any sedation or general anesthesia while receiving MRI. Conclusion: Using distractive methods or being close to parents should decrease the anxiety of the children while receiving MRI. With babies providing the physiological needs such as f eeding should be eff ective to decrease the need of sedation or general anesthesia requirement in MRI procedures.

Özet

Amaç: Çocuklarda, manyetik rezonans görüntülemeyi (MRG) yüksek nitelikli olarak sonuçlandırmak gürültü ve çocukların hareketsiz kalma zorlukları nedeniyle her olguda olanaklı değildir. Bu çalışmanın amacı, çocukların MRG cihazındaki kooperasyonunu sağlamak amacıyla, bebeklerde f izyoloj ik gereksinimler karşılandıktan sonra MRG'ye alınmanın; çocuklarda dikkat dağıtıcı girişimlerin ve ebeveyne yakın olmanın sedasyon veya genel anestezi uygulama sıklığına etkisini araştırmaktır. Yöntem: Bu retrospetif çalışmada, yaşları 1 ay-4 yıl arasında olan, herhangi bir nedenle MRG istenen 60 hastanın dosyaları dahil edildi. MRG' ye alınmadan önce çocukların kulaklarına gürültüden etkilenmelerini azaltmak amacıyla pamuk tıkandı. Anneler MRG cihazının içine çocuğuyla birlikte kabul edildi. Oyuncaklarla veya bazı yiyeceklerle çocukların dikkatleri dağıtılarak hareketsiz kalmaları amaçlandı. Küçük (<12 ay) bebekler açlığı takiben emzirilerek uyutulduktan sonra MRG cihazına alındı. Yukarıdaki girişimlerin yetersiz kaldığı durumlarda, çocuklar sedatize edildi veya genel anestezi uygulandı. Bulgular: Çalışmamızda yaşı 12 aydan küçük olan 5 bebek vardı ve bu bebeklerin tamamı emzirme sonrası uyutularak MRG' ye alındı. Yaşları 1 yıldan daha büyük olan çocukların 36'sında (% 65.5) MRG çocuk uyanıkken yürütüldü ve sorunsuz olarak sonuçlandırıldı. Çocukların 7'sinde (% 12.7) sedasyon ve 12'sinde (% 21.8) genel anestezi uygulanması gerekti. Hastaların %65.5inde sedasyon veya genel anestezi uygulanmadı. Sonuç: Bebeklerde f izyoloj ik gereksinimler karşılandıktan sonra uyutularak MRG' ye alınmanın; çocuklarda dikkat dağıtıcı girişimlerin ve ebeveyne yakın olmanın sedasyon veya genel anestezi uygulama sıklığını azalttığını düşündürmektedir.
