RETROSPECTIVE ANALYSIS OF THE COMPLICATIONS OF THE SUBCLAVIAN VEIN CATHETERIZATIONS PERFORMED UNDER ENDOVASCULAR ELECTROCARDIOGRAPHIC GUIDANCE IN HACETTEPE UNIVERSITY NEUROSURGERY OPERATING THEATRES

Abstract
Central vein catheterization enables the monitorisation of central venous pressure, management of fluid administration andaspiration of venous air embolus from the entrance of right atrium. Accurate positioning of the catheter tip is essential to avoide complications.ECG guidance is a well-known technique for this purpose. In this retrospective study, we reported 1017 catheterizations in respect tocomplications and the effectiveness.Methods: After the approval of Hacettepe University Non-Interventional Researches Ethics Commitee, the reports of 1017 subclavianvein catheterisations done over a 9 years period (January 2004 to October 2013) in patients over 18, were retrospectively evaluated andresearch has been done on the matter if the experience of the person doing the application has an affect on the complications or not.Results: Between January 2004 and October 2013, 1364 subclavian vein catheterizations were performed of which 178 were under18 years old. One hundred and sixty-nine of them were not evaluated because of incomplete records. Nine hundred and sixty-two catheterizationswere performed with endovascular electrocardiographic guidence. The overall complication rate was 4.91%. One patient had pneumothorax(0.09 %) and 5 patients had arterial puncture (0.5%). Six patients had catheter tip malpositions (0.58%). Amongst 109 of the patients nosign of arithmia was observed during the catheterisation. The average number of needle insertion for specialists is 1.19±0.52 and for theresidents is 1.20±0.55 and the difference between them is not statistically significant (p=0.674). Amongst the specialists and residents,the ratio of catheterisation to be done by a more experienced staff is found to be statistically significant (p=0.007).Conclusion: Endovascular electrocardiogram is found effective for accurate positioning of subclavian vein catheter tip. Numberof attempts and complications were independent of the experience of the performer. As the anesthesiologists' experience increases, the needto leave the catheterization to a more experienced one decreases

Santral ven kateterizasyonu; santral venöz basıncın izlenmesine, sıvı yönetiminin düzenlenmesine ve venöz hava embolisininsağ atrium girişinden aspire edilmesine olanak sağlar. Kateter ucunun doğru yerleşimi; kateterin efektif kullanımı, santral venöz basıncındoğru ölçülmesi ve komplikasyonlardan kaçınılması açısından vazgeçilmezdir. İntrakardiyak elektrokardiyografinin, kateter ucunun doğruyerleştirilmesindeki etkinliğini ve kateter takılması sırasında oluşabilecek komplikasyon oranına etkisini araştırmak amacı ile subklavyanven kateterizasyon kayıtları incelenmiştir.Yöntem: Hacettepe Üniversitesi Girişimsel Olmayan Araştırmalar Etik Kurulu onayı alındıktan sonra hasta kayıtlarından üç ameliyathenededokuz yıllık süre boyunca (Ocak 2004-Ekim 2013) 18 yaş üstü hastalara uygulanan 1017 subklavyan ven kateterizasyonu kayıtları incelenmiştir.İşlemi gerçekleştiren kişinin tecrübesinin komplikasyonlar üzerine etkisi olup olmadığı araştırılmıştır. Bulgular: Ocak 2004-Ekim 2013 arasında toplam 1364 subklavyan ven kateterizasyonu uygulanmış, 178 hasta 18 yaş altı olması, 169hasta kayıtlardaki eksiklikler nedeni ile çalışma dışı bırakılmıştır. Endovasküler elektrokardiyografi kılavuzluğunda 962 kateterizasyonuygulanmıştır. Toplam komplikasyon oranı %4.91 bulunmuştur. Bir hastada pnömotoraks (%0.09), 5 hastada arteriyel ponksiyon (%0.5)kaydedilmiştir. Altı hastada kateter ucu malpozisyonu tespit edilmiştir (%0.58). Yüz dokuz hastada kateterizasyon sırasında aritmi gözlenmemiştir.Uzman ve asistanların iğne giriş sayılarının ortalamaları sırasıyla 1.19±0.52 ve 1.20±0.55'tir, aralarındaki fark istatistiksel olarak anlamlıdeğildir (p=0.674). Uzman ve asistanlar arasında, kateterizasyonun daha tecrübeli birine bırakılma oranı istatistiksel olarak anlamlıbulunmuştur (p=0.007). Sonuç: Bu çalışmada endovasküler elektrokardiyografi, subklavyan ven kateterizasyonunda kateter ucunun yerinin doğru belirlenmesindeetkili bulunmuştur. Bu çalışmaya ve literatürde yayınlanan bilgilere göre subklavyan ven kateterizasyonunun en sık komplikasyonu subklavyanarter ponksiyonudur. İğne girişi sayısı ve komplikasyon oranları uygulayıcının tecrübesinden bağımsız bulunmuştur. Uygulayıcının tecrübesiarttıkça kateterizasyonu daha tecrübeli bir anesteziste devretme ihtiyacı azalmaktadır
