Evaluation of relation between esophageal clearance and gastroesophageal reflux by pH monitoring in children

Abstract
Aim: A prospective study was performed to evaluate the relation between esophageal clearance (EC) and gastroesophageal reflux (GER) with pH monitoring (PM) in children. Methods: Between January-June 2004, cases whom we performed PM were enrolled into the study. Cases with less than 4% reflux index were grouped as non-reflux, and greater than 4% was the reflux group. Both of the groups were investigated for age, sex, reflux index, number of episodes, number of episodes longer than 5 minutes and EC. Results: 38 cases were included (non-reflux: 15, reflux: 23). Both groups were homogeneous for age and sex (non-reflux: 3.93±3.08, male/female: 6/9, reflux: 6.84±4.5, male/female: 8/15). According to PM findings; mean reflux index (non-reflux: 1.66±0.45, reflux: 18.17±3.4), number of episodes (non-reflux: 29.53±3.02, reflux: 144.6±52.5) and number of episodes longer than 5 minutes (non-reflux: 0.6, min:0-max:2, reflux: 9.65, min:0-max:58) were significantly increased in reflux group (p<0.05). EC was 1.14±0.65 min/reflux in the non-reflux group and did not differ from reflux group values (1.46±0.9 min/reflux). Although there was no relation between EC and reflux index and episodes longer than 5 minutes in the reflux group, EC was correlated reversely with the number of reflux episodes (p=0.01). Conclusion: EC which is detected by PM is 1.14±0.65 min/reflux in children without clinical reflux. There is no direct relation between EC and GER. EC is not affected by reflux time but affected by the number of reflux.

Amaç: Özefagus klirensinin (ÖK) gastroözefagial reflüyle (GÖR) olan ilişkisini değerlendirmek amacıyla GÖR düşünülen hastalar 24 saat pH monitorizasyon (PM) denetiminde ileriye dönük olarak incelenmiştir. Yöntem: 2004 Ocak-Haziran aylarında PM yapılan olgular GÖR indeksi % 4'ün altında olanlar GÖR'süz, % 4'ün üzerinde olanlar ise GÖR'lü grubu oluşturmak üzere yaş, cinsiyet, GÖR indeksi, epizot sayısı, 5 dakikadan uzun GÖR sayısı ve özefagus klirensi açısından incelenmiştir.Bulgular: Çalışmaya 15'i GÖR'süz ve 23'ü GÖR'lü olmak üzere toplam 38 olgu dahil edilmiştir . Yaş ve cinsiyet açısından gruplar homojendir (GÖR'süz: erkek/kız:6/9, yaş: 3.93±3.08 yıl, GÖR'lü: erkek/kız:8/15, yaş: 6.48±4.5 yıl). PM bulguları incelendiğinde GÖR'lü grupta ortalama GÖR' indeksi (GÖR'süz: 1.66±0.45, GÖR'lü: 18.17±3.4), epizot sayısı (GÖR'süz: 29.53±3.02, GÖR'lü: 144.6±52.5) ve 5 dakikadan uzun epizot sayısı (GÖR'süz: 0.6, min:0- maks:2, GÖR'lü: 9.65, min:0- maks:58) anlamlı yüksektir (p< 0.05). ÖK açısından GÖR'süz (1.14 ± 0.65 dk/reflü) ve GÖR'lü grup (1.46 ± 0.9 dk/GÖR) arasında belirgin fark gösterilememiştir. ÖK ve GÖR parametreleri karşılaştırıldığında ÖK ile GÖR indeksi ve 5 dakikadan uzun epizot sayısı arasında ilişki bulunmamakla birlikte epizot sayısı ile ÖK arasında ters korelasyon saptanmıştır (p=0.01). Sonuçlar: GÖR olmayan çocuklarda PM ile tespit edilen özefagus klirensi ortalama 1.14 ± 0.65 dk/reflüdür. ÖK ile GÖR arasında doğrudan bir ilişki bulunmamaktadır. ÖK, GÖR süresinden değil sayısından etkilenmektedir.
