Is decompression before contrast enema misleading in newborns with the suspicion of Hirschsprung's disease?

Abstract
Aim: Contrast enema is routinely used in the diagnosis of cases with a suspicion of Hirschsprung's disease (HD). The diagnostic findings of this procedure are; rectum with normal calibration or a distal narrow segment, funnel shaped dilatation in the transitional zone (TZ) and marked dilatation in the proximal colon. The traditional knowledge is that, decompression before enema may remove these findings and causes false negative results. In this study, the effect of a short duration of decompression prior to contrast enema is analyzed whether it makes any changes on the graphical findings or not. Material and Method: Twenty-four newborns and infants operated because of HD between 2001 and 2005 were retrospectively analyzed for gender, weight at birth and at operation, complaints and physical examination findings on time of admission, presence of decompression before contrast enema, diagnostic procedures and operative findings. Results: Girls/boys were 2/22. 21 patients had not passed stool in the first 24 hours. 21 patients had abdominal distension and 9 had bilious vomiting. Plain x-ray revealed air-fluid levels without any pelvic gas in 16 and marked gas distension in 5 patients. 3 patients had normal findings. Average weight at birth was 3190 (2200-4300) grs and 3450 (2400-4500) grs at operation. Mean age at operation was 22,5 (3-60) days. All patients had contrast enema before operation. Average duration of decompression with warm saline enema (2 x 10 cc/kg) and rectal tube (6/day) was 2,3 days. TZ was apparent in 21 (87.5 %) patients but could not be detected in 2 (8.33 %). 1 (4.16 %) patient had microcolon . Primary TEP was performed in 19 (79 %) patients. Laparotomy assisted TEP at single session was performed for patients in whom TZ could not be detected in 2 (8.33 %), microcolon was detected in 1 (4.16 %) and descending colon was involved in 2 (8.33 %). An average of 11,75 (5-30) cm proximal of the peritoneal reflection is anastomozed to the anus. Conclusion: The results of this study support that decompression with a short duration before any contrast enema does not affect the presence of TZ. Hence, decompression can be performed in newborns having abdominal distension and high risk of perforation.

Amaç: Hirschsprung hastalığı (HH) düşünülen olgularda baryumlu lavman (BL) rutin olarak uygulanmaktadır. Bu olgularda normal veya dar distal segment, transizyon bölgesinde huni şekilli dilatasyon ve proksimal kolonda belirgin dilatasyon kolon grafisinde saptanan bulgulardır. Klasik bilgi, kolon grafisi öncesi dekompresyon yapılmasının bu bulguların ortadan kalkmasına ve yalancı negatif sonuçlara neden olabileceği şeklindedir. Çalışmamızda kolon grafisi öncesinde yapılan kısa süreli dekompresyonun kolon grafisindeki bulguları etkileyip etkilemediğini geriye dönük olarak incelenmiştir. Gereç ve Yöntem: 2002-2005 yılları arasında kliniğimizde HH nedeniyle opere edilen 24 yenidoğan (YD) ve infant; cinsiyet, ortalama doğum ve operasyon kilosu, başvuru şikayetleri ve fizik muayene bulguları, KE öncesi dekompresyon yapılıp yapılmadığı, tanı yöntemleri ve operasyona ait bilgiler yönünden geriye dönük olarak incelemeye alındı. Bulgular: Hastaların; kız/erkek oranı: 2/22 idi. 21 hastada, ilk 24 saatte mekonyum çıkışı olmamıştı. 21 hastada karın şişliği, 9 hastada ise safralı kusma şikayeti vardı. Ayakta direkt batın grafisinde (ADBG) 16 hastada hava-sıvı seviyeleri vardı ve pelvik gaz yoktu. Hastaların 5'inde yoğun gaz birikimi varken, 3 hastanın grafisi normaldi. Ortalama doğum kilosu: 3190gr (2200-4300), operasyon kilosu: 3450 gr. (2400-4500), operasyon yaşı 22,5gün (3-60). Tüm hastalara operasyon öncesi tanı amaçlı Baryumlu kolon grafisi çekildi. Kolon grafisi öncesi ortalama 2,3 gün (1-5), günde 2 kez 10 cc/kg ılık serum fizyolojik lavman ve günde 6 kez rektal tüp ile dekompresyon uygulanmıştı. 21 hastada (% 87.5) transizyonel zon (TZ) saptanırken 2 hastada (% 8.33) TZ gösterilemedi. 1 hastada (% 4.16) ise mikrokolon vardı. Hastaların 19 tanesine (% 79) primer TEP, TZ'u gösterilemeyen 2 hasta (% 8.33), mikrokolona sahip 1 hasta (% 4.16) ve inen kolon tutulumu olan 2 hastaya (% 8.33) laparotomi yardımlı TEP tek seansta uygulandı. Peritoneal refleksiyonun ortalama 11,75 cm proksimali (5-30) anüse anastomoze edildi. Sonuç: Bu çalışma, BL öncesi yapılan kısa süreli dekompresyonun TZ'un gösterilmesini engellemediğini ve perforasyon riski olan, batın distansiyonlu yenidoğan ve infant olgularda KE öncesi kısa süreli dekompresyon uygulanabileceğini düşündürmektedir.
