Streptococcus pyogenes Pneumonia with Toxic Shock Syndrome

Abstract
Streptococcal toxic shock syndrome (STSC) is a rapidly progressive and mortal condition that develops mainly due to invasive Streptococcus pyogenes (S. pyogenes) infections. In this report, it was aimed to present a 45-year-old male patient with a history of diabetes mellitus (DM) who was admitted to the emergency department with complaints of fever, cough, sputum and dyspnea. On admission, his fever was 38°C, blood pressure was 100/60 mmHg, and chest respiratory rales were heard over the right middle lung fields on auscultation. Laboratory tests on admission showed elevated leukocyte count, BUN (blood urea nitrogen), creatinine and lactate values, and posteroanterior (PA) chest X-ray and lung computed tomography (CT) findings were consistent with right lung middle lobe pneumonia. Intravenous fluid, norepinephrine infusion, piperacillin-tazobactam and clarithromycin combination were started as antibiotherapy. S. pyogenes was isolated in the sputum culture and in both of the two sets of blood cultures taken at the admission of the patient. On the third day of hospitalization, he was intubated due to the development of acute respiratory distress syndrome (ARDS), and hemodialysis was performed due to the development of anuria, and the patient died on the fourth day of treatment with shock and multiorgan failure. S. pyogenes is not a common factor in community-acquired pneumonia. STSS has a very similar clinical picture with sepsis and septic shock associated with other bacteria. Despite appropriate treatment, it is considered to be a condition that should be kept in mind because of its high mortality rate and rapid clinical course. This case is presented to remind uncommon STSS.
Özet
Streptokoksik toksik şok sendromu (STŞS) özellikle invaziv Streptococcus pyogenes infeksiyonlarına bağlı olarak gelişen hızlı seyirli vemortal bir tablodur. Bu raporda diabetes mellitus (DM) öyküsü olan, ateş, öksürük, balgam ve nefes darlığı şikayetleri ile acil servisebaşvuran 45 yaşında erkek hasta sunulmaktadır. Hastanın başvurusunda ateşinin 38°C, tansiyonunun 100/60 mmHg olduğu sağakciğer orta zonda ince ralleri olduğu saptandı. Laboratuvar bulgularında; lökosit, BUN, kreatinin ve laktat değerlerinin yüksek olduğu,posteroanterior (PA) akciğer grafisi ve akciğer bilgisayarlı tomografi (BT) bulgularının sağ akciğer orta lob pnömonisi ile uyumlu olduğu görüldü. İntravenöz sıvı ve norepinefrin infüzyonu ile antibiyoterapi olarak piperasilin-tazobaktam ve klaritromisin kombinasyonubaşlanan hastanın kabulünde alınmış olan balgam kültüründe ve iki set kan kültürünün ikisinde de S. pyogenes izole edildi, tedavisineyatışının ikinci gününde klindamisin eklendi. Yatışının üçüncü gününde akut solunum sıkıntısı sendromu (ARDS) gelişmesi nedeniyleentübe edilen ve anüri gelişmesi nedeniyle hemodiyaliz uygulanan hasta şok ve multiorgan yetmezliği ile tedavisinin dördüncü günündekaybedildi. S. pyogenes toplum kökenli pnömonilerde sık görülen bir etken olarak karşımıza çıkmamaktadır. STŞS ise diğer bakterilerleilişkili sepsis ve septik şok ile oldukça benzer bir klinik tablo sergilemektedir. Uygun tedaviye rağmen yüksek mortalite oranı ve hızlıklinik seyri nedeniyle akılda tutulması gereken bir tablo olduğu düşünülmektedir. Bu olgu, yaygın olarak görülmeyen STŞS’yi hatırlatmakamacıyla sunulmuştur
