Use of metoclopramide and ondansetron for prevention of postoperative nausea and vomiting in outpatients operated in otorhinolaringology

Abstract
The study was performed in Otorhinolaringology Department of Ankara University Faculty of Medicine on 90 children, ages between 2-12, operated for adenoidectomy and/or tonsillectomy. Depending on their cooperation, the patients were given inhalation anesthetics with a mask or intravenous agent for anesthesia induction. 1,5 mglkg succinylcholine was used for intubation. Just after the intubation patients were divided randomly into three groups. For prophlaxy of emesis, 0,1 mglkg ondansetron to the first group, 2 ml normal saline to the second group and 0,25 mglkg mtoclopramide to the third group was administered. Stewad's (17) Par Scale was used for assessment of recovery of the patients. Although the score were lower in metocloprami-de group than the other groups, this was not found to be statistically significant. The rate of vomiting was 10% in ondansetron group, 40% in control group and 35% in metoclopramide group. The differen¬ces between the first and the second, and, the first and the third groups were statistically significant (p < 0,05). We concluded that ondansetron is more effective in prophlaxy of postoperative nausea and vomiting than metoclopramide.

Çalışma Ankara Üniversitesi Tıp Fakültesi kulak Burun Boğaz Hastalıkları Kliniğinde adenoidektomi ve/veya tonsil- lektomi operasyonu geçiren, 2-12 yaşları arasında 90 çocuk üzerinde uygulandı, kooperasyonlanna göre, hastalara maske ile veya intravenöz genel anestetiklerle indüksiyon sağlandı. 7,5 mgökg süksinilkolin kullanılarak, entübasyon yapıldı. Entü- basyondan hemen sonra, rasgele üç gruba ayrılan hastalardan birinci gruba antiemetik proflaksi için 0,1 mglkg ondanset- ron, ikinci gruba 2 mi serum fizyolojik, üçüncü gruba da 0,25 mglkg metoklopramid uygulandı. Steward'in (17) PAR skorlamasına göre hastalarda derlenme karakteristikleri takip edildi. Metoklopramid grubunda, diğer iki gruba göre daha düşük skorlar elde edildi; ancak istatisitksel anlamlılık göstermedi. Ondansetron grubunda, %10 kusma gözlenriken, bu oran kontrol grubunda %40 ve metoklopramid grubunda da %35 olarak bulundu. Birinci ile ikinci ve birinci ile üçüncü grup arasındaki fark istatistiksel olarak da anlamlı idi (p < 0,05). Postoperatif bulantı kusma proflaksisinde ondansetronun daha etkili olduğu sonucuna varıldı.
