Complications of functional endoscopic sinus surgery

Abstract
Functional endoscopic sinus surgery (FESS) has become an increasingly popular treatment for chronic para-nasal sinus infections. In this study minor complications of 50 patients undergoing FESS at Başkent University ENT Clinic is reported. The most frequent complications were fracture of the lamina paprycea and postoperative synechia between lateral nasal wall and middle concha. Other problems included intraoperative bleeding, persistent or recurrent polyposis. 15 pati¬ents operated under local anesthesia and general anesthesia was used for 35 patients after which the surgicall site was injec¬ted with Articain. Modified Stammberger (Messerklinger) approach was used as a surgicall technique. If a deviated septum was revealed, septoplasty was performed before FESS was started. At the conclusion of the procedure a silastic sheet (2x4 cm.) was placed in the middle meatus between lateral nasal wall and middle concha to prevent synechia. Revision FESS was indicated in 3 patients within 6 months of the initial surgery. The most common cause of revision FESS was due to synechia resulting ostiomeatal occlusion. To avoid synechia we recommed placing a silastic sheet in to the middle meatus at the medd¬le meatus at the end of surgery.

Kronik paranazal sinüs enfeksiyonlarının tedavisinde fonksiyonel endos'kopik sinüs cerrahisi (FESC) son yıllarda gi¬derek artan bir popülariteye sahiptir. Bu çalışmamızda Başkent Üniversitesi KBB kliniğinde gerçekleştirilen 50 FESC'ndeki karşılaşılan minör komplikasyonlar sunulmuştur. En sık rastlanılan komplikasyonlar lamina paprycea kırığı ve postoperatif burun lateral duvarı ile orta konka arasında gelişen sineşiler olmuştur. Bunun yanışım intraoperatif kanama, persistan ve rekürren polip olguları karşılaşılan diğer problemlerdir. Onbeş olgu lokal anestezi altında, 35 olgu genel anestezi altında operasyon bölgesine articain ile ek anestezi uygulanarak öpere edilmişlerdir. Cerrahi teknik olarak Stammberger'in tarifledi-ği modifiye Messerklinger yaklaşımı kullanılmıştır. Hastalarda septum deviasyonu mevcut ise öncelikle septoplasti gerçek¬leştirilerek FESC uygulanmıştır. Operasyon bitiminde orta konka ile lateral burun duvarı arasında gelişebilecek sineşişi önlemek amacı ile 2x4 cm.lik silastik levha orta meaya yerleştirilmiştir. Postoperatif takipleri esnasında 3 olguya revizyon cerrahisi endikasyonu koyulmuştur. Revizyonun en önemli sebebi postoperatif osteomeatal kompleksde sinesi gelişmesidir. Operasyon bitiminde orta meaya konacak silastik levhanın sinesi ve rekürrens şansını azaltacağı görüşündeyiz.
