Personal Protective Equipment Use Otorhinolaryngologist in COVID-19 Pandemics in the Light of a Case with Advanced Head and Neck Cancer, Carotid Rupture and COVID-19

Abstract

Coronavirus disease-2019 (COVID-19) pandemics began in China in December 2019. The first case was diagnosed in Turkey on the March 11, 2020, and the number of cases increased rapidly. Physicians and other healthcare professionals treating COVID-19 patients are among the riskiest groups in terms of getting infected with the virus. Although personal protective equipment (PPE) that healthcare workers other than otorhinolaryngologists should use in different contact situations with the patient has been determined by Ministry of Health, ear nose throat (ENT) examination has a special place since it involves upper respiratory tract, requires close contact with the patient, and the patient cannot wear a mask during the examination. Therefore, ENT physicians’ PPE need is different from other specialties. In this article, appropriate PPE for ENT physicians has been discussed in the light of an advanced stage of laryngeal cancer patient who had a carotid rupture and was diagnosed with COVID-19 2 days after intervention, and the literature regarding asymptomatic/presymptomatic cases. It was concluded that ENT physicians should use level 3 PPE while working in the outpatient and inpatient facilities and emergency procedures in order to protect themselves from COVID-19 disease transmission.

Özet

Koronavirüs hastalığı-2019 [coronavirus disease-2019 (COVID19)] pandemisi Aralık 2019 tarihinde Çin’de başlamış olup, Türkiye’deise ilk vakaya 11 Mart 2020 tarihinde tanı konmuş ve vaka sayıları hızlaartmıştır. COVID-19 hastalarını tedavi eden hekimler ve diğer sağlıkçalışanları, tüm dünyada virüse yakalanma açısından en riskli gruplararasındadır. Sağlık Bakanlığı tarafından; hastayla yakın temas içerendurumlarda kulak burun boğaz (KBB) muayenesi yapmayacak olan sağlık çalışanlarının kullanması gereken kişisel koruyucu ekipman(KKE)lar belirlenmiştir. KBB muayenesi üst solunum yolunu içerdiğinden, hastayla yakın temas gerekmekte ve muayene sırasında hastayamaske takmak mümkün olmadığı için, KBB hekimlerinin muayene vediğer işlemler sırasında kullanması gereken KKE’ler özellik arz etmektedir. Bu çalışmada, ileri evre bir larinks kanseri olgusunda ortaya çıkankarotis rüptürüne müdahale süreci ve bu sırada KBB ekibinin KKE kullanma durumu ele alınmıştır. Müdahaleden 2 gün sonra COVID-19 pozitif olduğu anlaşılan bu olgunun ve asemptomatik/presemptomatikolgulara ilişkin literatür ışığında, KBB hekimlerinin kullanması gereken uygun KKE’ler gözden geçirilmiştir. Pandemi sürecinde KBB hekimlerinin, kendilerini ve çevrelerini COVID-19 hastalığı bulaşındankorumak için rutin poliklinik, servis ve acil işlemlerini yürütürken seviye3 KKE kullanmaları gerektiği sonucuna varılmıştır.
