Management of Dysphagia During COVID-19 Pandemic: Gazi University Faculty of Medicine ENT Clinical Experience

Abstract
Uncertainties remain regarding the effect of coronavirus disease-2019 (COVID-19) on the health system and our overall life. For this reason the COVID-19 epidemic has forced it necessary to change the health services and care provided to patients all over the world. As in all other health services, it is important to establish operational steps in the management of patients with swallowing disorders, in order to increase efficiency. Swallowing assessment is one of the procedures at high risk of transmission of the virus. However, COVID19 significantly affect the pulmonary system, a possible aspiration pneumonia due to swallowing disorder may lead to further aggravation of the current condition. Many studies during the COVID-19 pandemic have emphasized the importance of telehealth services, which can be used to minimize the risk of cross contamination from close contact. Therefore, the swallowing functions of these patients should be evaluated remotely as far as possible, but when face-to-face assessment is required or any airway procedures such as flexible endoscopy or nasogastric tube are used, using personal protective equipment (PPE) according to the highest risk of aerosol contamination should be taken. The procedure, including the FFP2 (N95) mask, goggles, gloves and apron, and full face protection should be followed. It is also important how to properly put on and take off these personal PPEs to prevent contamination during and after the procedure.

Yeni tip koronavirüs hastalığı [coronavirus disease-2019(COVID-19)]’nın sağlık sistemi ve genel yaşamımız üzerindeki etkisinin ne kadar süreceği konusunda belirsizlikler devam etmektedir. Bunedenle COVID-19 salgını, tüm dünyada hastalara sunulan sağlık hizmet ve bakımlarının değişmesini zorunlu kılmıştır. Diğer tüm sağlık hizmetlerinde olduğu gibi, yutma bozukluğu olan hastaların yönetiminde deişleyiş basamaklarının oluşturulması verimin artırılması açısından önemlidir. Yutma değerlendirmesi, virüsün bulaş riski yüksek olan prosedürlerdendir. Bununla birlikte COVID-19 pulmoner sistemi önemli ölçüdeetkiler. Yutma bozukluğuna bağlı gelişebilecek olası bir aspirasyon pnömonisi, mevcut tablonun daha da ağırlaşmasına yol açabilir. COVID19 pandemisi süresince yapılan birçok çalışmada, yakın temas kaynaklıçapraz kontaminasyon riskini en aza indirmek için kullanılabilecek olantelesağlık hizmetlerinin önemi vurgulanmıştır. Bu sebeple hastalarınyutma fonksiyonları olabildiğince uzaktan değerlendirilmeli. Ancak yüzyüze değerlendirme gerektiğinde ya da esnek endoskopi veya nazogastrik tüp gibi herhangi bir hava yolu prosedürünün uygulanması söz konusu olduğunda, en üst düzey aerosol bulaş riskine göre kişisel koruyucuekipmanlar (KKE) kullanılarak önlem alınmalıdır. FFP2 (N95) maskesi,koruyucu gözlük, eldiven ve önlük ile tam yüz koruması içeren prosedür uygulanmalıdır. İşlem sırasında ve sonrasında kendi kendine kontaminasyonu engellemek için bu KKE’lerin giyilip çıkarılırken gerekenkurallara uyulması son derece önemlidir.
