The prognostic effects of the clinic / histopathologic parameters and P53 expression in carcinomas of parotid gland

Abstract
To investigate the prognostic affects of histopathologic and clinical criteria, as well as p53 expression, on loco-regional recurrences and distant metastasis, a study in a group of patients (n-52) hat was operated with diagnosis of carcino¬ma of parotid gland, between the years 1968 and 1996, in Hacettepe University Medical School Otorhinolaryngology De¬partment., is present. P53 expression was detected in 44.2% of the patients and found to be a positive prognostic factor. It was also concluded that, in order to investigate the prognostic significance of p53 expression in different histological subg¬roups of patients, studies with higher numbers of cases must be planned.
Özet
Çalışmamızda Hacettepe Üniversitesi Tıp Fakültesi Kulak Burun Boğaz Anabilim Dalına 1968 - 1996 tarihleri ara¬sında başvurarak parotis karsinomu tanısıyla ameliyat olmuş 52 hasta incelenmiştir. Hastalara ait histopatolojik materyal¬lerde immünhistokimyasal yöntem ile p53 tümör supressör geni malfonksiyonu araştırılmıştır. Parotis karsinomlarında prognoz üzerinde etkili olabilecek histopatolojik ve kilink kriterler ile p53 pozitifliği ele alınarak, lokal rekürrens ve uzak me¬tastaz üzerine etkileri ortaya konulmuştur. p53 pozitifliği %44.2 gibi yüksek bir oranda olmuştur. İleri evre ve agresif seyirli tümörlerde daha düşük oranda p53 pozitifliği saptanmıştır. p53 pozitifliği iyi prognoz göstergesi olarak bulunmuştur. Ancak, parotis kanserlerinin değişik prognoza sahip alt gruplarında farklı oranlarda p53 pozitifliği saptanması nedeniyle, p53 overekspresyonunun prognoz üzerine etkilerinin ortaya konulmasında tümör tek tek ele alındığı, daha geniş serili çalış¬maların yapılması gerektiği kanaatine varılmıştır.
