Importance of mastoid pneumatization on secretory otitis media

Abstract

The secretory otitis media is the most common middle ear disease in childhood. It heals spontaneously, by me¬dical therapy or by minor surgical procedures in most of the cases. Sequelae such as retraction pockets and adhesive otitis that lead to cholesteatoma can rarely occur, but at the beginning it is hard to estimte which patient will acquire a sequela. It is well know that mastoid pneumatization is poor in the complications like retraction pocket, adhesive otitis and cholesterol granuloma. In this study, mastoid X-rays of 28 children with secretory otitis media were evaluated. Twenty-two of them who needed venti¬lation tubes had a second X-ray. Mastoid volume results of both groups were compared. We concluded that mastoid pneumatization might he considered as a prognostic indicator in secretory otitis media.

Özet

Sekretuar otitis media (SOM) çocukluk döneminde en sık görülen bir orta kulak hastalığıdır. Bu hastaların büyük bir kısmında hastalık spontan, medikal tedavi veya minör cerrahi girişimlerle düzelmekte, az bir kısmında ise kolesteatomun ön¬cesi olan retraksiyon poşu, adeziv otitis gibi sekeller meydana gelebilmektedir. Ancak hangi hastada ileride sekel bırakacağı ilk anda bilinmemektedir. Retraksiyon poşu, adeziv otit ve kolesterol granülom gibi SOM komplikasyonlarında mastoid tam gelişmemiş ve havalanması yetersiz olduğu iyi bilinmektedir. Bu görüşten yola çıkarak yaptığımız çalışmada kliniğimize başvuran 28 SOM'lı çocuk hastaya lateral mastoid graftsi (Schüller graft) çektirdik. Ventilasyon tüpü takılan 22 hastaya 6 ay sonra grafileri tekrarladık, ameliyat öncesi-sonrası so¬nuçlarla ve medikal tedaviyle düzelen 6 hastanın mastoid volümlerini birbirleriyle karşılaştırdık ve mastoid pnömotizasyo-nunun SOM'da prognostik bir faktör olabileceği sonucuna vardık.
