The place of contralateral selective neck dissection in head and neck oncologic surgery

Abstract
Objective: To determine the role of contralateral selective neck dissection in head and neck oncologic surgery. Patients and Methods: This study included patients operated at our clinic for primary larynx or oral cavity squamous cell carcinoma with NO and N1 lymph node involvement in the neck. Besides primary resection of the tumor and ipsilateral neck dissection, we performed a contralateral selective neck dissection to all these patients. The results were evaluated prospectively for pathological lymph node involvement and and development of neck recurrences. Results: This study included 28 patients. There were 2 females and 26 males with a mean age of 55 (36-75 years). The mean follow-up time was 45,3 months (6-62 months). Ipsilateral radical neck dissection, ipsilateral modified neck dissection and ipsilateral selective nek dissectino were performed to 9, 18, 1 patients respectively and simultaneous contralateral selective neck dissection was performed to all patients. Metastatic lymph node involvement was detected in ipsilateral neck of 8 patients (29%) and contralateral neck of 5 patients (18%). There were 5 cases (18%) who only had ipsilateral positive nodes and 2 cases (7%) who only had contralateral positive nodes, 3 cases (12%) had positive nodes on both sides of the neck. 2 of the cases (%25) with ipsilateral neck metastasis developed ipsilateral neck recurrence ater surgery. One NO patient developed contralateral neck recurrence. Conclusion: Contralateral selective neck dissection is necessary for treatment of contralateral occult metastasis and for planning of postoperative adjuvant therapy.

Amaç: Bu çalışmada, baş boyun onkolojik cerrahisinde kontralateral selektif boyun diseksiyonunun endikasyonları ve sonuçları incelenmiştir. Hastalar ve yöntemler: Ocak 1996 ve Kasım 2000 arasında kliniğimizde primer larinks veya oral kavite yassı hücreli karsinomu nedeniyle ameliyat edilen hastalar dahil edildi. Evrelendirme sonucu NO ve Ni olan olgulara primer lezyonun rezeksiyonu ve ipsilateral boyun diseksiyonunun yanı sıra kontralateral boyun diseksiyonu da uygulandı. Boyun spesimenlerindeki patolojik lenf nodu tutulumu ve nüks gelişimi prospektif olarak değerlendirildi. Bulgular: Çalışmaya toplam 28 olgu dahil edildi. Olguların 2'si kadın ve 26'sı erkek olup ortalama yaş 55 (36-75) idi. Olguların ortalama takip süresi 45,3 ay (6-62) idi. Olgulardan 9'una ipsilateral radikal, 18'ine ipsilateral modifiye radikal, birine ipsilateral selektif boyun diseksiyonu uygulandı. Bunun yanı sıra, tüm hastalara kontralateral selektif boyun diseksiyonu uygulandı. 8 olguda (%29) ipsilateral, 5 olguda (%18) kontralateral lenf nodu metastazı bulundu, sadece ipsilateral lenf nodu metastazı bulunan 5 olgu (%18), sadece kontralateral lenf nodu metastazı bulunan 2 olgu ( %7) ve bilateral lenf nodu metastazı bulunan 3 olgu (%12) saptandı. İpsilateral boyun metastazı bulunan olguların 2'sinde (% 25) boyunda nüks gelişmiştir. Bir vakada ise patolojik olarak NO olmasına rağmen kontralateral boyunda nüks gelişmiştir. Sonuç: Kontralateral selektif boyun diseksiyonu kontralateral okkult metastazın ortaya çıkarılması için ve postoperatif yardımcı tedavinin belirlenmesi için gereklidir.
