Serdetection of Epstein-Barr virus (EBV) DNA with insitu hybridization (ISH) method: In nasopharengeal carcinoma (NPC), Burkitt and non-Burkitt lymphomas

Abstract

Acute EBV is a DNA virus that infects oropharengeal epithelial cells and B Imyphocytes. Insitu hybridization (ISH) provides detection of cellular and viral DNA or RNA nucleic acid sequences. EBV is known to be involved in the ethio-pathogenesis ofNPCs and some B and T cell lymphomas. In the current study we tried to investigate the presence of EBV DNA using ISH method, in 4 NPCs, 11 Burkitt lymphomas (BL), 6 diffuse large cell lymphomas (DLCL), 2 lymphoblastic lymphomas (LL) and I immunoblastic lymphoma (IL) that were sent to our department between the years of 1990-1994. We detected EBV DNA in 13(54.1%) of the cases. Positive staining showed nuclear localization. In 1 (25%) of the NPCs diffuse, nuclear staining was seen in all of the cells. We detected positivity in 6 (54.5%) of BLs and in 6 of (66.6%) ofnon BLs. Among the EBV (+) DLCLs 3 were CD20 (+), 1 was CD 45RO (+) and 1 was CD 20(-). CD 45RO (-). LL which was EBV (+) was also found to be CD 45RO (+), Though the positivity in NPC materials was low, the number of cases was not enough to make a definitive conclusion. The quality of our tissues, the type of probe we used and the number of target viral genome which could be lower than detectable copies might be effective on our results. The positivity rates in our BLs were lower than endemic cases but higher than sporadic ones. Since BL cases in Turkey make an "intermediate" group clinically and EBV prevalence is high in normal population, this finding t not surprising. Most of the positive cases in our non BLs showed B cell feature which was consistent with B cell tropism of EBV. Recently presence EBV in some T cell lymphomas has been re¬ported in both nasolnasopharengeal and abdominal locations

Akut Epstein - Ban viüs (EBV) orofarenksin epitel hücrelerini veB lenfositleri enfekte edebilen bir DNA virüsüdür. Insitu hibridizasyon (İSH) hücresel ya da viral DNA veya RNA'nın nükleik asit sekansının saptanabilmesini sağlar. EBV'ün nazofar.enks karsinomlari (NFK), bazı BveT hücreli lenfornalarda etiyopatogenezde rol aldığı bilinmektedir. Bu çalışmada 1990-1994 yılları arasında bölümümüze gelen 4 NFK, 11 Burkitt lenfoma, 2 lenfoblastik lenfoma, 6 diffüz büyük hücreli len-foma ve 1 immünoblastik lenfomada İSH yöntemiyle EBV varlığını araştırdık. Vakalarımızın 13'de (%54.1) EBV-DNA'yı saptadık. Pozitif boyanma nükleer lokalizasyon gösteriyordu. NFK'larının l'inde (%25) yaygın, hemen tüm nükleuslarda boyanma mevcuttu. Burkitt lenfoma vakalarımızın 6'sında (%54.5), Burkitt dışı lenfomalarımızın yine 6'sında (%66.6) pozitiflik saptadık. EBV gösterilebilen diffüz büyük hücreli lenfomaların 3'ü CD20 (+), l'i CD 45RO (+), l'i de hem CD20(-) hem CD 45RO (-) idi. EBV-DNA (+) lenfoblastik lenfoma vakamız ise CD 45RO (+) olarak belirlendi. NFK materyallerimizde aldığımız sonuçlar düşük olmakla birlikte kesin bir yorum yapmak için vaka sayımız yetersizdir. Bunda doku kalitemiz, kullandığımız probun türü ve hedef viral genomun saptanabilir düzeyin altında olması etken olabilir. Burkitt lenfomalardaki oranımız endemik olanlardan düşük, sporadik vakalardan yüksektir. Bulgumuz Türkiye'deki Burkitt'lerin klinik olarak "intermediate" bir grup oluşturdukları ve normal populasyonda EBV insidansımn yüksek olduğu düşünülürse çok şaşırtıcı değildir. Non-Burkitt lenfoma hasta grubumuzun çoğu B hücre özelliği göstermekteydi ki EBV'ün B hücre tropizmi bilinmektedir. T hücreli lenfo-malarda EBV varlığı ise son yıllarda hem nazolnazofarengeal hem de abdominal lokalizasyonlarda yayınlanmıştır.
