Cancer of the tongue: Experience with 88 cases

Abstract

Although tongue cancer is the second most frequent oral cavity cancer, its treatment has been controversial. In this study, the authors present their experience with surgical, treatment of tongue cancer, 88 patients were evaluated retrospectively. There were 39 T1, 27 T2, 12 T3, 10 T4 lesions. Clinical neck evaluation was N0 for 58 patients. N1 for 22, N2 for 7, and N3 for 1 patient . For early and anteriorly located lesions pull through operation was chosen. For advanced and posteriorly located lesions commando type operation was the treatment of choice. For the ipsilateral neck 59 radical, 14 modified radical, 13 supramohyoid and 2 selective (Level 1, 2, 3, 4) neck dissections were done. For the contralateral neck 2 radical, 4 modified radical, 3 supraomohyoid and 17 suprahyoid neck dissections were performed. The overall occult metastasis rate was 29.3 % (17/58), and the occult metastasis rate for T1 and T2 lesions was 21.7%% (810/46). Among the contralateral neck dissections, metastasis, was detected for 2 cases. 44 patients were treated with surgery only. 26 patients received postoperative radiotherapy, 13 received postoperative chemotherapy, 5 patients were given both postoperative radiotherapy and chemotherapy, 24 patients experienced recurrences, 16 of them were local, 7 regional recurrences, and 1 distant metastasis .3 of the regional recurrences were in the contralateral undissected neck. The overall 3 year-disease-free- survival rate was 78.8 %% for T1 and T2 lesions, and 54.5 % for T3 and T4 lesions. As a conclusion. The rate of occult metastasis to the neck is too high in all tongue cancer cases to take the risk of regional recurrence. Radiological investigations and fine needle aspiration decrease. But never diminish the rate of false negative examination. There is an obvious indication for neck dissection even in early cases.

Dil kanseri, oral kavitenin en sık görülen ikinci kanseri olmasına rağmen, tedavisi hala tartışmalıdır. Bu araştırma, kliniğimizin dil kanserinin cerrahi tedavisindeki tecrübesini aktarmaktadır. 88 hasta retrospektif olarak değerlendirilmiştir. Hastaların 39'u T1, 27'si T2, 12'si T3, 10'u T4 olduğu görülmüştür. Boyun klinik değerlendirilmesi, 58 hastada N0, 22 hastada N1, 7 hastada N2 ve 1 hastada da N3 olarak belirlenmiştir. Küçük ve önde lokalize lezyonlar için pull-through ameliyatı yapılmıştır. Ileri evre ve arkada yerleşmiş lezyonlar için ise commando ameliyatı uygulanmıştır. Ipsilateral boyun için 59 radikal, 14 modifiye radikal, 13 supraomohyoid ve 2 selektif (Level 1-2-3-4) boyun diseksiyonu yapılmıştır. Kontralateral boyun için 2 radikal, 3 supraomohyoid 17 suprahyoid boyun diseksiyonu uygulanmıştır. Okült metastaz oranı % 29.3 (17/58) ve t1-2 lezyondaki okült metastaz oranı ise % 21.7 (10/46) bulunmuştur. İki kontralateral borun diseksiyununun metastaz saptanmıştır. 44 hastaya sadece cerrahi, 26 hastaya cerrahi+ postoperatif radyoterapi, 13 hastaya cerrahi + postoperatif kemoterapi, 5 hastaya da cerrahi+ postoperatif radyoterapi +kemoterapi uygulanmıştır. 24 hasta da rekürrens görülmüştür. Bunların 16'sı lokal, 7'si rejyonel, 1'i de uzak metastazdır. Rejyonel rekürrenslerin 3'ü diseke edilmemiş kontralateral boyunda görülmüştür. 3 yıllık hastalıksız yaşam oranı T1 ve T2 hastalar için % 78.8 T3 ve T4 lezyonlar için ise %54.5 bulunmuştur. Sonuç olarak, tüm dil kanseri hastalarında okült metastaz oranı çok yüksektir. Radyolojik incelemeler ve ince iğne aspirasyonu muayenedeki yalancı negatif oranını düşürmekle birlikte, hiçbir zaman ortadan kaldırmamaktadır. Erken lezyonlarda bile, boyun diseksiyonu yapılması gerektiği kanaatine varılmıştır.
