Infra-parotid open reduction technique for the fractures of the mandibular condyle

Abstract
The open reduction for the fractures of the mandibular condyle has technical difficulties in the exposure and fixation of bone fragments, and carries risks of complications. This article defines the surgical technique of an infra-parotid approach to expose the mandibular ramus and condyle, and reports the early outcome in five patients. Based on the principle of wide exposure gained by systematic dissection and retraction of the main trunk and servicofacial branch of the facial nerve and the external carotid artery, this approach provided effective open reduction and fixation. It was concluded that, the infraparotid approach is a reliable technique for the open reduction of fractures of the mandibular condyle, which minimizes the risks for permanent facial nerve paralysis.

Mandibula kondil kırıklarının açık redüksiyonu, kırık fragmalarının manüplasyonunda ve fiksasyonunda teknik güçlükler ve komplikasyon riskleri taşır. Bu makalede, mandibula kondil kırıklı beş hastada uygulanan infra-parotid yöntem tanımlanmış ve erken sonuçları bildirilmiştir. Fasial sinir ana trunkusu ile servikofasial dallarının ve eksternal karotid arterin sistematik diseksiyonu ve ekartasyonu ile geniş görüş alanı sağlanması prensibine dayanan bu yöntem, iki hastada geçici ve inkomplet fasial paralizi dışında komplikasyon olmaksızın mandibula kondil kırıklarının etkili redüksiyonunu ve rijid fiksasyonunu sağlamıştır. Bu tekniğin, mandibula kondil kırıklarının açık redüksiyonunda ve rijid fiksasyonunda kalıcı fasial sinir hasarı riskini asgariye indirgeyen güvenilir bir yöntem olduğu sonucuna varılmıştır.
