Ethmoid sinus microbiology in patients with chronic sinusitis: (A new method for transnasal sampling)

Abstract
Chronic sinusitis is one of the most frequently encountered chronic diseases. Therapy of the disease is first medical, and in case of unsuccessfulness, surgical intervention may be performed. Empirical antibiotics are preferred agents for the medical treatment since obtaining cultures from the sinuses is an invasive procedure and usually it is difficult to obtain cultures in the office. The selection of the empirical antibiotics is based upon previous microbiological studies on paranasal sinus microbiology. In this study, we used a technique that minimizes the risk of contamination while obtaining ethmoid sinus cultures and investigated the ethmoid sinus microbiology on 60 sides of 42 patients who underwent functional endoscopic sinus surgery (FESS) for chronic sinusitis. Bacterial growth was observed in 56.7 % of the samples. Multibacterial growth was evident in 14.7% of the samples. The most frequently encountered pathogenic agent was S. Aureus (20.5%) followed by Bacteriodes spp (12.8%) Anaerobes alone or with aerobes were isolated in 29.4 % of ethmoid sinuses. Strep, epidermidis was isolated in only 3.3% of the cultures and this suggested the prevention of the contamination with the use of our technique.
Özet
Kronik sinüzit, en sık görülen kronik hastalıklardan birisidir. Tedavisi önce medikal, başarılı olunamazsa, cerrahidir. Medikal tedavi başlıca antibiyotiklerle yapılır. Sinüslerden kültür almak invaziv bir işlem olduğu ve poliklinik şartlarında genellikle mümkün olmadığı için, kronik sinüzitli hastalara ampirik antibiyotik tedavisi verilmektedir. Ampirik tedavide de kronik sinüzitli hastaların sinüs mikrobiyolojilerini belirleyen çalışmalar yol gösterici olmaktadır. Kliniğimizde kronik sinüzit nedeniyle FESC uygulanan 42 hastanın 60 tarafından kontaminasyonu büyük derecede önlediği düşünülen yeni bir yöntemle kültür alındı. Alınan tüm kültürlerin %56.7'sinde üreme oldu. Kültürlerin 5'inde Ureyen S. aureus ilk sıradaydı, ikinci sıklıkta (%12.8) Bacteriodes spp. üredi. Kültürlerin %29.4'ünde aeroblarla birlikte veya tek başına anaerob bakteriler üredi. Strep epidermidis kültürlerin sadece %3.3'ünde üredi ve bu sonuç, kültür alma yöntemimizin büyük ölçüde kontaminasyonu elimine ettiğini düşündürdü.
