The Effect of Spinal Anesthesia on Cochlear Functions

Abstract
Objective: The aim of this study was to investigate the effect of spinal anesthesia on cochlear function. Material and Methods: This study included 36 American Society of Anesthesiologists I-II patients between 18-50 years and who had undergone spinal anesthesia in the urology clinic with having no contraindications for spinal anesthesia. The patients had no ear problems before. Distortion product otoacoustic emissions (DPOAE) were performed three times (preoperative, postoperative 1st day and postoperative 7th day) for each patient with normal preoperative pure tone audiometry and tympanometry. The signal-to-noise ratio (SNR) at 1000, 1500, 2000, 3000, 4000, 6000 and 8000 Hz frequency results were compared with each other. Results: No significant difference were observed between the DPOAE signal-to-noise ratios (SNR) measured preoperative and on days 1 and 7 after spinal anesthesia for both right and left ears (p>0.05). Only one patient had sudden hearing loss at low frequencies on the first postoperative day. Intravenous steroid treatment was administered to the patient. On the 7th postoperative day, hearing loss improved. Conclusion: Hearing loss is an important complication of spinal anesthesia. Therefore both the anesthesiologist and the surgeon should inform the patient about this possible complication.

Amaç: Spinal anestezinin koklear fonksiyonlara etkisinin incelenmesidir. Gereç ve Yöntemler: Bu çalışmaya, üroloji kliniğindespinal anestezi ile opere olan, spinal anestezi açısından kontrendikasyonu olmayan, Amerikan Anestezistler Derneği I-II, yaş aralığı 18-50yıl olan, işitme kaybı oluşturacak herhangi bir kulak problemi olmayan 36 hasta dâhil edildi. Preoperatif saf ses odyometrileri (SSO) vetimpanometrileri normal olan hastalara preoperatif, postoperatif 1. günve postoperatif 7. gün distortion product otoakustik emisyon (DPOAE)yapıldı. 1000, 1500, 2000, 3000, 4000, 6000 ve 8000 Hz frekanslarındaölçülen sinyal gürültü oranları [signal-to-noise ratios (SNR) preoperatif, postoperatif 1. gün ve postoperatif 7. gün birbirleriyle mukayeseedildi. Bulgular: Hastaların preoperatif, postoperatif 1. gün ve postoperatif 7. gün DPOAE 1000, 1500, 2000, 3000, 4000, 6000 ve 8000 Hzfrekanslarında SNR sonuçları karşılaştırıldığında hem sağ hem sol kulakta istatistiksel anlamlı farklılık izlenmedi (p>0,05). Bir hastada isepostoperatif 1. günde alçak frekanslarda belirgin olan ani işitme kaybıgelişti. Hastaya intravenöz steroid tedavisi düzenlendi. Postoperatif 7.günde işitme kaybının iyileştiği görüldü. Sonuç: İşitme kaybı, spinalanestezinin önemli komplikasyonlarındandır. Gerek anestezi uzmanı,gerekse cerrahi işlemi yapacak olan hekimin bu olası komplikasyonaçısından hastayı öncesinde bilgilendirmesi önemlidir.
