PETROZ APEKS PNÖMATİZASYONLU TEMPORAL KEMİKLERDE ANTERİOR EPİTİMPANİK RESES VE COG ÇIKINTISININ ÜÇ BOYUTLU MORFOMETRİK DEĞERLENDİRMESİ

Abstract
Amaç: Bu kesitsel, radyo-anatomik araştırmanın amacı, petröz apeks pnömatizasyonu ile Cog çıkıntısı uzunluğu ve anterior epitimpanik reses (AER) hacmi arasındaki ilişkiyi temporal kemiğin 3 boyutlu rekonstrüktif bilgisayarlı tomografi kesitlerini kullanarak değerlendirmektir. Metod: OsiriX yazılımıyla rekonstrükte edilen üç boyutlu modellerden anterior epitimpanik reses hacmi ölçüldü. Petroz apeks pnömatizasyonlu ve pnömatizasyonsuz temporal kemiklerin Cog çıkıntısı uzunlukları ile AER hacimleri karşılaştırıldı. Bulgular: Petroz apeks pnömatizasyonu olan temporal kemiklerde Cog uzunluğu, olmayan kemiklere göre anlamlı olarak daha düşük olup AER hacmi anlamlı olarak yüksekti (sırasıyla p = 0.01 ve p <0.001). Sonuç: Petroz apeksin pnömatizasyonu, epitympanumun daha iyi havalanması nedeniyle retraksiyon cebi gelişimine karşı fizyolojik bir avantaj sağlayabilecek şekilde daha geniş bir AER ve daha kısa Cog çıkıntısı ile ilişkili bulundu.
Özet
Purpose: To assess the association of petrous apex pneumatization with Cog process length and anterior epitympanic recess volumeusing 3- dimensional reconstructed computerized tomography sections of the temporal bone, in this cross- sectional, radio-anatomicalinvestigation.Method: We measured anterior epitympanic recess volumes from three- dimensional models reconstructed on OsiriX software. Thecomparisons of Cog process lengths and anterior epitympanic recess volumes of the temporal bones with and without petrous apexpneumatization were performed.Results: Cog process length was significantly lower, and AER volume was significantly higher in temporal bones with petrous apexpneumatization compared to the bones without (p=0.01 and p<0.001, respectively).Conclusion: Pneumatization of petrous apex was found associated with a wider AER and shorter Cog process that might bring about aphysiological advantage against retraction pocket development because of well- pneumatized epitympanum.
