A retrospective evaluation of pediatric cases hospitalized due to intoxication

Abstract

Intoxication remains a serious health problem and can have se- rious consequences, depending on the substance received or the time elapsing to presentation to hospital. Some patients presen- ting to emergency departments due to intoxication are hospita- lized for treatment, even in intensive care units. The purpose of this study was to evaluate demographic characteristics, causes of intoxication, and information concerning length of hospitali- zation and course of disease in cases of childhood poisoning pre- senting to the emergency department. Medical records of pati- ents with intoxication aged 0-18 years presenting to the Erzurum Regional Training and Research Hospital and receiving inpatient treatment between 2008, and 2016 were evaluated retrospecti- vely. A total of 642 cases were evaluated within the scope of the study. The mean age of the cases was 8.1±6.6 years (mean±SD). Causes of intoxication were drugs in 57.9% (n=372), pesticides in 11.5% (n=74), cleaning products in 8.6% (n=55), and wild plants in 3.3% (n=21) of the cases, while the cause was unknown in 18.7% (n=120) of the patients. The mean length of hospitaliza- tion was 3 days, and 9% (n=58) of the patients received inpati- ent treatment in the intensive care unit. A psychopathology was determined in 21% (n=135) of the cases, and major depressive disorder being identified in 85.9% (n=116) of them. According to our findings, the most important part of childhood poisoning is poisonings that accidentally occur under 5 years of age. In children over 13 years of age, poisonings and psychopathologies are associated with suicide. Drugs are the most frequently used agent in poisoning. Treatment for intensive care is needed for %9 of the patients.

Zehirlenme ciddi bir sağlık sorunu olarak önemini korumakta, alınan maddeye veya hastaneye başvuru süresine bağlı olarak ciddi sonuçlar doğurabilmektedir. Zehirlenme nedeniyle acil ser- vislere başvuran hastaların bir kısmı, yoğun bakım dahil yatarak tedavi görmektedir. Bu çalışmada acil servise başvuran ve yata- rak tedavi gören çocuk zehirlenmelerinde, olguların demografik özellikleri, zehirlenme nedenleri, yatış süresi ve hastalık seyrine ait bilgilerin değerlendirilmesi amaçlandı. Erzurum Bölge Eğitim ve Araştırma Hastanesine 2008-2016 tarihleri arasında başvuran ve yatarak tedavi gören 0-18 yaş zehirlenme olgularına ait tıbbi kayıtlar retrospektif olarak değerlendirildi. Çalışma kapsamın- da 642 olgu değerlendirildi. Ortalama yaş (mean±SD) 8,1±6,6 idi. Olguların zehirlenme nedenleri sırasıyla; %57,9’u (n=372) ilaçlar, %11,5’i (n=74) pestisitler, %8,6’sı (n=55) temizlik mad- deleri, %3,3’ü (n=21) yabani bitki olup, %18,7 (n=120) olgunun nedeni bilinmiyordu. Olguların hastanede ortalama yatış süre- si 3 gün olup, %9’u (n=58) yoğun bakımda yatarak tedavi gör- müştü. Olguların %21’inde (n=135) bir psikopatoloji saptanmış olup, bunlardan %85,9’unda (n=116) majör depresif bozukluk tespit edilmiştir. Bulgularımıza göre yatarak tedavi gören çocuk zehirlenmelerinin önemli kısmını 5 yaş altında kazara meydana gelen zehirlenmeler oluşturmaktadır. On üç yaş üstü çocuklarda ise özkıyıma bağlı zehirlenmeler ve psikopatolojiler ön plandadır. İlaçlar, zehirlenme etkeni olarak ilk sıradadır. Olguların %9’unda yoğun bakımda tedaviye gereksinim duyulmaktadır.
