Otological approach in residual acoustic neurinoma: A case report

Abstract
The retrosigmoid approach, which presumably provides a good exposure to pontocerebellar angle tumors, may not be adequate in cases where the tumor shows supratentorial extension to the middle cranial fossa. In this report, a 57-year-old woman is presented, in whom the residual tumor in the temporal lobe was subsequently removed by the middle fossa approach following acoustic neuroma surgery through the retrosigmoid approach. No residual tumor was detected postoperatively, and no recurrences were encountered during a five-year follow-up.

Pontoserebellar köşe açısından çok geniş görüş alanı sağladığı savunulan retrosigmoid yaklaşım, tümörün supratentoriyel ilerleme ile orta kafa çukuruna yaydım gösterdiği durumlarda yeterli görüş alanı sağlayamaz. Akustik nörinom tanısı ile nöroşirürjenler tarafından retrosigmoid varış yoluyla ameliyat edilen 57 yaşındaki bir kadın hastada temporal lobdaki tümör kalıntısı, orta kafa çukuru yaklaşımı ile çıkarıldı. Ameliyat sonrasında tümör kalıntısı saptanmayan hastanın beş yıllık takibinde nükse rastlanmadı.
