Level 1 dissection for laryngeal cancer: Is it necessary ?

Abstract
Objectives: To determine the prevalence of the involvement of the first level in patients undergoing laryngectomy and neck dissection for squamous cell carcinoma of the larynx. Patients and Methods: The records of 154 patients (150 men, 4 women; mean age 54 years) who underwent laryngectomy and neck dissection for squamous cell carcinoma of the larynx were reviewed with respect to involvement of the first level. There were 230 surgical specimens dissected from 80 patients (n=119) with supraglottic, 22 patients (n=24) with glottic, and 52 patients (n=87) with transglottic tumors. Results: Involvement of level 1 was detected in five specimens, accounting for 2.2% and 3.2% among all neck dissections and laryngectomies, respectively. All the patients with level 1 involvement were found to have positive neck disease originating from the supraglottic larynx. Conclusion: Our findings suggest that inclusion of level 1 in neck dissections may not be necessary, especially in N0 necks.

Amaç: Larenks kanseri nedeniyle larenjektomi+ boyun diseksiyonu yapılan hastalarda tümör lokalizasyonu ile level 1 tutulumu arasındaki ilişki incelendi. Hastalar ve Yöntemler: Larenjektomi ve boyun diseksiyonuna Level 1'in dahil edildiği 154 olgunun (150 erkek, 4 kadın; ört. yaş 54) boyun diseksiyonu materyalleri (n=230 ) retrospektif olarak incelendi. Olguların 80'inde (n=119) supraglottik, 22'sinde glottik (n=24) ve 52'sinde (n=87) transglottik tutulum vardı. Bulgular: Boyun diseksiyonu örneklerinin beşinde level 1 tutulumu saptandı. Tüm boyun diseksiyonları dikkate alındığında level 1 tutulum oranı %2.2, larenjektomi uygulanan hasta sayısı dikkate alındığında ise %3.2 bulundu. Level 1 tutulumu saptanan olguların tümünün supraglottik yerleşimli, N(+) boyunlu hastalar olduğu görüldü. Sonuç: Larenks kanseri tedavisinde, özellikle N0 boyunlarda boyun diseksiyonlarına level 1 'in katılımının gereksiz olduğu sonucuna varıldı.
