A case of congenital petrous apex cholesteatoma

Abstract
A thirty-one year-old male patient presented with facial paralysis, pain in the left ear, hearing loss, and tinnitus. He had received medical therapy for paralysis of the left peripheral facial nerve 4.5 years ago, but no improvement had been obtained. A diagnosis of congenital cholesteatoma of the petrous apex was made following physical examination and investigations with computed tomography and magnetic resonance angiogra-phy. Under general anesthesia, he underwent subtotal petrosectomy by posterior and anterior translabyrinthine approaches with preservation of the otic capsule. During a two-year follow up, no recurrences or complications were seen except facial paralysis due to preoperative cholesteatoma compression, which was treated by hypoglossal-facial nerve anastomosis.
Özet
Otuz bir yaşında erkek hasta, yüz felci, sol kulak ağrısı, işitme azlığı ve çınlama yakınmasıyla başvurdu. Hastada, 4.5 yıl önce sol periferik fasyal sinir paralizisi tanısıyla medikal tedavi uygulanmasına rağmen düzelme sağlanmamıştı. Muayene, bilgisayarlı tomografi ve manyetik rezonans anjiyografi tetkikleri sonucunda petroz apeks konjenital kolesteatomu tanısı kondu. Genel anestezi altında subtotal petrosektomi; otik kapsül korunarak, kolesteatomun yaygınlığı nedeniyle posterior ve anterior translabirentin yaklaşımlar uygulandı. Yaklaşık iki yıllık takip sırasında hastada ameliyat öncesindeki kolesteatom kompresyonuna bağlı fasyal paralizi dışında hiçbir nüks belirtisi ve başka komplikasyon görülmedi. Fasyal paralizide düzelme olmaması üzerine hipoglossofasyal sinir anastomozu uygulandı.
