Therapeutic use of Botulinum toxin A in a patient with Frey syndrome

Abstract

A twenty-seven-year-old female patient who had undergone superficial parptidectomy for pleomorphic adenoma developed Frey's syndrome eight months after surgery. The diagnosis was confirmed with the use of the Minor's iodine-starch test. Botulinum toxin type A (2.5 U/cm2) was injected intradermally to the affected sites. Her symptoms disappeared two days after the injection and repeat Minor's test performed after three weeks showed a dramatic decrease in the sweating areas. However, her symptoms recurred eight months after the injection and the Minor's test revealed areas of sweating. Botulinum toxin A was injected once more at the same dose. Three weeks after the second injection, the Minor's test showed complete disappearance of the sweating areas.

Özet

Pleomorfik adenom nedeniyle yüzeyel parotidektomi uygulanan 27 yaşındaki kadın hastada ameliyattan sekiz ay sonra Frey sendromu gelişti. Minör testi ile tanısı kesinleştirilen hastaya intrakutan olarak 2.5 U/cm2 botulinum toksin A infiltrasyonu yapıldı. Enjeksiyondan iki gün sonra semptomların tamamına yakınının kaybolduğu; üç hafta sonra yapılan Minör testinde terleme alanlarının neredeyse tümüyle kaybolduğu gözlendi. Hastanın semptomlarının sekiz ay sonra tekrarlaması üzerine yapılan Minör testinde Frey sendromunun nüks ettiği görüldü. Botulinum toksin A enjeksiyonu aynı dozda tekrarlandı. Üç hafta sonraki Minör testi terleme alanlarının tamamen kaybolduğunu gösterdi.
