Oncologic and functional results in head and neck reconstruction with radial forearm free flaps

Abstract
Objectives: To evaluate our functional and oncologic results of microvascular free flaps in head and neck reconstruction. Patients and Methods: Radial forearm free flaps were used in the reconstruction of 12 patients (4 men, 8 women; age range 37 to 63 years) with advanced hypopharyngeal tumors (n=5) or oral cavity cancers (n=7). Reconstruction was performed during the same session for primary tumor resection in 10 patients; two patients underwent secondary reconstruction. Patients were evaluated with regard to reconstruction technique, early and late complications, wound healing, flap viability, oral feeding and articulation, and disease-free survival. Results: Total flap loss occurred in only one patient on the postoperative forth day. Mean duration for oral feeding was 15 days and 9.5 days in patients with hypopharyngeal and intraoral reconstruction, respectively. Articulation and deglutition were satisfactory in patients undergoing intraoral free flap reconstruction. Upper digestive tract reconstruction using designed radial forearm free flaps, performed immediately in three patients and at the second stage in two patients, provided early oral feeding and successful defect closure. Four patients died due to tumor-related complications. One patient is still alive with his tumor. None of the patients had local recurrences except for one patient with another primary tumor. Conclusion: Radial forearm free flap, which is thin, pliable, and reliable with low morbidity, appears ideal for intraoral and hypopharyngeal reconstructions.

Amaç: Baş boyun rekonstrüksiyonunda uyguladığımız mikrovasküler serbest fleplerin fonksiyonel ve onkolojik sonuçlarını değerlendirmek. Hastalar ve Yöntemler: İleri evre ağız içi (n=7) ve hipofarenks (n=5) tümörü olan 12 hastanın (4 kadın, 8 erkek; yaş dağılımı 37-63) rekonstrüksiyonunda radial ön kol serbest flep kullanıldı. On olgunun rekonstrüksiyonu, primer tümör rezeksiyonu ile aynı seansta yapıldı. İki olguda sekonder doku defekti onarıldı. Tüm olgular uygulanan rekonstrüksiyon tekniği, erken ve geç dönem gözlenen komplikasyonlar, yara iyileşmesi, flep canlılığı, oral beslenme-artikülasyon ve hastalıksız dönem yönünden değerlendirildi.Bulgular: Sadece bir hastada operasyon sonrası dördüncü günde tam flep kaybı meydana geldi. Ağız yolu ile beslenmeye başlama için geçen ortalama süre hipofarenks rekonstrüksiyonu yapılan hastalarda 15 gün, ağız içi rekonstrüksiyonu yapılan hastalarda 9.5 gündü. Ağız içi rekonstrüksiyonu yapılan hastalarda artikülasyon ve yutkunma tatmin edici bulundu. Aynı seansta üç hastada ve ikinci seansta iki hastada şekil verilerek yapılan flep uygulaması ile üst sindirim yolunun rekonstrüksiyonu, erken ağız yolu ile beslenme ve başarılı bir defekt onarımı sağlandı. Dört hasta tümör nedeniyle kaybedildi; bir hasta tümörü ile yaşıyor. İkinci primer tümörü olan bir hasta dışında lokal nüks gelişmedi.Sonuç: Radial ön kol serbest flepler, İnce, kıvrılabilir, güvenilir ve düşük morbiditeli olması nedeniyle, oral ve hipofarenks rekonstrüksiyonunda en iyi seçenektir.
