Anatomical comparison of retrosigmoid and retrolabyrinthine approaches to the acousticofacial pedicle

Abstract
Objectives: The posterior fossa approach is a commonly used surgical method, utilizing two basic route to approach the posterior cranial fossa, namely, the retrosigmoid and the retrolabyrinthine routes. This study investigated particularly the acousticofacial pedicle in relation to each anatomical structure in the posterior fossa and the surgeon's view. Design and Methods: Six cadaveric temporal bone: were surgically dissected, using the retrosigmoid (n=3) and the retrolabyrinthine (n=3) approaches. Operation time, difficulty in exposing the posterior fosse structures, and the influence of different types of lesions on each procedure were assessed. Results: Operation time in the retrosigmoid approach was threefold less than that of the retrolabyrinthine approach. The risk of injury to the vertical segment of the facial nerve and posterior semicircular canal was greater in the retrolabyrinthine approach. Identification of the seventh and eighth nerves was better in the retrosigmoid approach. Conclusion: The retrosigmoid approach was found superior to the retrolabyrinthine approach.
Özet
Amaç: Posterior fossa yaklaşımı günümüzde sık kullanılan cerrahi yöntemlerden biridir. Bu sahaya yaklaşımda kullanılan iki temel yol retrosigmoid ve retrolabirenter yoldur. Bu çalışmada, özellikle akustikofasyal pedikül ile her iki yaklaşımda karşılaşılan posterior fossadaki anatomik yapılar arasındaki ilişkiler ve cerraha göre konumlar incelenmiştir. Çalışma Planı ve Yöntemler: Altı kadavra temporal kemiğinin üçünde retrosigmoid, üçünde retrolabirenter yaklaşım uygulandı. Diseksiyonlarda her bir yaklaşım için operasyon süresi, posterior fossa yapılarının açığa çıkarılmasındaki zorluk ve farklı patolojilerin uygulanan yaklaşımı etkileme oranı değerlendirildi. Bulgular: Retmosigmoid yaklaşımda operasyon süresi, retrolabirenter yaklaşıma göre üç kat daha kısaydı. Fasyal sinir vertikal segmenti ve posterior semisirküler kanalın yaralanma riski retrolabirenter yaklaşımda daha fazlaydı. Yedinci ve sekizinci sinirlerin ayırımının retrosigmoid yaklaşımda daha kolay olduğu görüldü. Sonuç: Retrosigmoid yaklaşım, retrolabirenter yaklaşımdan daha üstün bulunmuştur.
