Hemostatic effects of bismuth subgallate in tonsillectomy

Abstract
Objectives: Hemorrhage is the most important complication of tonsillectomy. In this prospective study, we investigated the hemostatic effects of bismuth subgallate in tonsillectomy. Patients and Methods: The study included 102 patients (46 girls, 56 boys; mean age 6.8 years; range 3-14 years) who underwent tonsillectomy for chronic tonsillitis. Tonsillectomy was performed using cold dissection and the snare technique. Patients were randomly assigned to control (n=54) and bismuth (n=48) groups, respectively. Postoperatively, all patients were evaluated on days 2, 4, 7, 10, and 15. The number of gauze packs and suture ligations used for hemostasis were compared. Results: The mean numbers of gauze packs were 9.22 and 4.65 in the control and bismuth groups, respectively (p<0.05). Suture ligations were required in 48 patients, with a mean number of 2.83 in the control group. In the bismuth group, nine patients required suture ligations with a mean number of 0.27 (p<0.05). Conclusion: Use of local bismuth subgallate in tonsillectomy proved to be an effective, safe, simple, and inexpensive method for hemostasis.

Amaç: Tonsillektominin en önemli komplikasyonu kanamadır. Bu prospektif çalışmada, lokal bizmut subgallat uygulamasının tonsillektomilerde hemostatik etkisini araştırdık. Hastalar ve Yöntemler: Çalışmaya kronik tonsillit tanısı ile tonsillektomi uygulanan 46'sı kız, 56'sı erkek 102 hasta alındı (ort. yaş 6.8; dağılım 3-14). Tonsillektomi genel anestezi altında künt diseksiyon ve ans yöntemiyle yapıldı. Rastgele yöntemle kontrol grubuna 54, bizmut grubuna 48 hasta ayrıldı Hastalar ameliyat sonrası dönemde 2, 4, 7, 10 ve 15. günlerde kanama açısından kontrol edildi ve çalışma ve kontrol gruplarında hemostaz için kullanılan tampon ve sütür sayıları karşılaştırıldı. Bulgular: Kontrol grubunda kullanılan ortalama tampon sayısı 9.22, bizmut grubunda 4.65 bulundu (p<0.05). Kontrol grubunda sütür gerektiren hasta sayısı 48, ortalama sütür sayısı 2.83 idi. Bizmut grubunda sütür gerektiren hasta sayısı dokuz, ortalama sütür sayısı 0.27 idi (p<0.05). Sonuç: Tonsillektomi sonrası lokal bizmut subgallat uygulamasının etkili, güvenilir, basit ve pahalı olmayan bir hemostaz yöntemi olduğu sonucuna varıldı.
