Voice restoration with Blom-Singer prosthesis in patients undergoing total laryngectomy

Abstract
Objective: We evaluated the results in patients in whom voice restoration was performed with Blom-Singer valve prosthesis following total laryngectomy. Patients and Methods: The study included 70 patients (11 women, 59 men; age range 38 to 80 years) who underwent voice restoration with Blom-Singer prosthesis following total laryngectomy. All patients had appropriate follow-up (mean 3 years 7 months). The type of prosthesis, durability, and complications were evaluated. Successful outcome was defined as the ability of the patient to make a telephone speech. Results: Sixty patients (%85) achieved successful speech. Average prosthesis lifetime was 6.8 and 7.1 months for low pressure (n=60) and indwelling prostheses (n=10), respectively (p>0.05). The most frequent complications were extrusion of prosthesis and closure of the tracheoesophageal puncture (21%). Conclusion: Voice restoration using Blom-Singer prosthesis is an easy procedure with a high success rate.

Amaç: Kliniğimizde total larenjektomi sonrası ses restorasyonu amacıyla Blom-Singer ses protezi uygulanan olgularda sonuçlar değerlendirildi. Hastalar ve Yöntemler: Çalışmaya total larenjektomi sonrası Blom-Singer protezi ile ses restorasyonu uygulanan 70 hasta (11 kadın, 59 erkek; yaş dağılımı 38-80) alındı. Tüm hastaların yeterli takipleri vardı (ort. takip süresi 3 yıl 7 ay). Kullanılan protezin tipi, ömrü ve komplikasyonlar değerlendirildi. Hastanın telefonda konuşabilmesi başarılı uygulama olarak kabul edildi. Bulgular: Altmış hastada (%85) başarılı konuşma elde edildi. Ortalama protez ömrü düşük basınçlı protezlerde (n=60) 6.8 ay, bakım gerektirmeyen protezlerde (n=10) 7.1 ay bulundu (p>0.05). En sık karşılaşılan komplikasyonlar protezin yerinden çıkması ve fistülün kapanması idi (%2.1). Sonuç: Total larenjektomi sonrasında ses restorasyonu için kullanılan Blom-Singer ses protezi iyi ses kalitesi sağlayan kolay uygulanabilir bir tekniktir.
