Ultrasonography-guided fine needle aspiration in the evaluation of nonpalpable lymph nodes in head and neck tumors

Abstract
Objectives: We evaluated the value of ultrasonog-raphy- (US) guided fine needle aspiration in nonpal-pable lymph nodes of the neck. Patients and Methods: Ultrasonography-guided fine needle aspiration was performed for 98 nonpalpable lymph nodes in 23 patients with head and neck tumors. The patients then underwent radical neck dissection. The findings were compared with histopathologic results. Results: The sensitivity, specificity, positive, and negative predictive values of US alone were 97%, 64%, 62%, and 97%, respectively. Ultrasonography-guided fine needle aspiration increased specificity to 96%. Sensitivity, positive and negative predictive values were 97%, 94%, and 98%, respectively. Conclusion: The specificity of US-guided fine nee-dle aspiration in nonpalpable lymph nodes in the neck is significantly higher than that of US alone. Routine application of US and US-guided fine nee-dle aspiration may influence the decision and the method of neck dissection in patients with head and neck tumors and nonpalpable lymph nodes.

Amaç: Çalışmamızda palpabl olmayan boyun lenf nodlarının değerlendirilmesinde ultrasonografi (US) eşliğinde ince iğne aspirasyonunun değeri araştırıldı. Hastalar ve Yöntemler: Baş-boyun tümörlü 23 has-tada US ile saptanan 98 palpabl olmayan lenf nodu-na US eşliğinde ince iğne aspirasyonu yapıldı. Bul-gular radikal boyun diseksiyonu sonrasında, histo-patolojik sonuçlarla karşılaştırıldı. Bulgular: Palpabl olmayan boyun lenf nodlarmda tek başına US'nin duyarlılığı %97, özgüllüğü %64, pozitif belirleme değeri %62, negatif belirleme değeri %97 idi. Ultrasonografi ile birlikte ince iğne aspirasyonunda özgüllük %96'ya yükseldi; duyarlılık, pozitif ve negatif belirleme değerleri sırasıyla %97, %94 ve %98 bulundu. Sonuç: Palpabl olmayan boyun lenf nodlarının değer-lendirilmesinde US eşliğinde ince iğne aspirasyonu özgüllüğü anlamlı derecede artırmaktadır. Baş-boyun tümörlü ve klinik olarak lenf nodu saptanmayan olgular-da, US ve US eşliğinde ince iğne aspirasyonunun bir-likte rutin olarak uygulanması, boyun diseksiyonu kararını veya diseksiyon şeklini etkileyecektir.
