The comparison of the exicion/marsupialization with exicion/primary suture in treatment of pilonidal sinus

Abstract

Objective: Pilonidal sinüs is common and chronic disease of sacrococcygeal area. Although there are many operative and non operative treatment techniques for pilonidal sinus, ideal treatment method is doubtfull. The results of two different treatment method is discussed in this study (exision/marsupialization (Gl) and. exicion/primary suture (G2)), and also other alternatives are rewieved in the literature. Materials and methods: The treatment methods were compared for the hospital stay time, postoperative complications, wound healing, and relapse retrospectively. Results: There were 40 patients (38 M, 2 F) in Gl and 39 patients (38 M, 1 F) in G2. The common complaints were sacrococcygeal pain (95%), purulant secretion (93%), and swelling (50%). The mean follow up period of the patients was 25.3+ 6.4 months (6-48 months). Postoperative early complications were seperation of the suture in 2 patients in Gl and wound infection in 9 patients in G2, the difference was significant (PO.0.5). There was not any significant difference for the hospital stay time in groups. The wound healing time was 29±9.2 days in Gl and 16±4.5 days in G2, the difference was significant ( p<0.05 ). The wound healing time was 36.2 ± 13.2 days in 9 patients who developed infection in G2 and this was significantly higher from Gl (p<0.05 ). Relaps were seen in one patient in Gl and in two patients in G2 and the difference was significant ( p<0.05). Conclusions: Recently there is not only one treatment option for pilonoidal sinüs. The choice of the treatment method must be indivudialized for each patient and the preferred technique must be performed with minor complication.

Özet

Amaç: Pilonidal sinus, sakrokoksigeal bölgenin kronik ve sık görülen hastalığıdır. Pilonidal sinüs tedavisinde çok sayıda operatif ve nonoperatif tekniğe rağmen ideal tedavi metodu tartışmalıdır. Bu çalışmada eksizyon ve marsupializasyon ( Gl ) ile eksizyon ve primer sütür ( G 2) uyguladığımız vakaların sonuçları tartışılmıştır. Literatürdeki diğer tedavi seçenekleri kısaca gözden geçirilmiştir. Gereç ve yöntemler:Bu çalışmada gruplar postoperatif komplikasyonlar, hastanede yatış süreleri, yara iyileşmesi ve nüks yönünden retrospektif olarak karşılaştırıldı. Bulgular:G1 hastaların 38'i erkek 2'si kadın, G2 hastaların 38'i erkek l'i kadındı. Hastaların en sık başvuru nedeni sakrokoksigeal bölgede ağrı ( % 95 ), pürülan akıntı ( % 93 ) ve şişlik ( %50 ) idi. Hastaların takip süresi ortalama 25.3 ±6.4 ay ( 6-48 ay ) idi. Ameliyat sonrası erken komplikasyon Gl'de 2 hastada sütür ayrılması, G2'de 9 hastada yara enfeksiyonu görüldü ve aradaki fark anlamlı (p<0.05) idi. Hastanede kalma süreleri açısından grublar arasında anlamlı bir fark yoktu. Yaranın kapanma süresi Gl'de 29±9.2 gün, G2'de ise 16±4.5 gün idi ve aralarındaki fark anlamlıydı ( p<0.05 ). G2'de enfeksiyon gelişen 9 hastada yaranın kapanma süresi ortalama 36.2± 13.2 gün olarak tespit edildi ve Gl'den anlamlı olarak yüksek idi ( p<0.05 ). Nüks Gl'de 1, G2'de ise 2 hastada görüldü ve fark anlamlı idi (p<0.05). Sonuç: Günümüzde pilonidal sinüs için tek bir tedavi seçeneği yoktur. Seçilecek tedavi yöntemi her hasta için ayrı değerlendirilmeli ve seçilen teknik minör komplikasyon ile uygulanmalıdır.
