The role of oxidative stress in patients with uncontrolled type II diabetes mellitus

Abstract

Objective: Type II Diabetes Mellitus (T II DM) is the most prevalent disease of endocrine disorders. Most of the patients are controlled inadequately. DM is associated with important complications such as vascular effects on collagen of vascular systems and is a potential biomarker of lipid derived oxidative stress. Our aim in this study was to evaluate the relationship between Malondialdehyde (MDA) and the complications of uncontrolled TII DM. Materials and methods: Twenty-eight uncontrolled TII DM outpatients had a long diabetic history and their levels of Hb Alc were higher than 7.0 % (Reference value was 4.0-6.0 %). MDA levels were measured by the modified Okawa method. Results: We found that MDA levels were significantly higher in uncontrolled group.(4.83±6.11 and 2.25± 0.39 nmol/ml) pO.001. Conclusions: MDA and oxidizied LDL are mediated pro-atherogenic processes. Our results confirm that MDA is a significant parameter for the late vascular system complications of uncontrolled T II DM. The glucose glycated collagen stimulates the oxidation of LDL and this results increase in MDA amounts.

Özet

Amaç: Tip II Diabetes mellitus (T II DM) en sık görülen endokrin hastalıktır. Hastaların birçoğu yetersiz kontrol altında bulunmaktadır. DM'ün önemli komplikasyonlarmdan birisi vasküler düz kasdaki kollajen üzerine olan ve lipid peroksidasyonu sonucu oluşan Malondialdehit (MDA) ile olmaktadır. Bu çalışmada MDA ile TII DM'un komplikasy onları arasındaki ilişkinin araştırılması amaçlandı. Gereç ve Yöntem: Uzun süreli T II DM hikayesi olan ve İç hastalıkları polikliniğine başvuran kan şekeri düzeyleri düzenli olmayan 27 hasta çalışma grubunu oluşturmaktaydı. Kontrol grubu olarak 17 sağlıklı olgu çalışma kapsamına alındı. MDA düzeyleri modifiye Okawa yöntemine göre spektrofotometrik olarak ölçüldü. Bulgular: Hasta grubunun Hb Alc düzeyleri % 7.0'nin üzerinde bulunmaktaydı. (Referans değerler: % 4.0-6.0) Hasta grubumuzdaki MDA düzeyleri kontrol grubuna oranla oldukça yüksek (4,83±6,11 ve 2,25±0,39 nmol/ml) (p<0.001) bulunmuştur. Sonuç: MDA ve okside LDL Kolesterol ateroskleroz öncesi devrede önemli role sahiptirler. Bulgularımız T II DM'daki vasküler geç komplikasyonları göstermesi açısından MDA ölçümünün önemli bir parametre olarak kullanılabileceğini düşündürmektedir. Glukoz, kollajeni glikozillemekte ve bu da LDL kolesterol'ün oksidasyonunu sağlayarak MDA düzeylerinde artışa neden olmaktadır.
