The Predictive Value of C-Reactive Protein to Albumin Ratio in Postoperative Atrial Fibrillation After Coronary Artery Bypass Grafting Surgery

Abstract

Objective: Atrial fibrillation is one of the most challenging postoperative complications after cardiac surgery. Occurrence of postopera‐ tive atrial fibrillation increases morbidity and mortality as well as increases health expenditures. Therefore, it is important to identify patients in the risk group and take precautions. The determination of the inflammatory condition that triggers atrial fibrillation before the operation contributes to the detection of these patients significantly. We aim this study, we examined the relationship between C‐ reactive protein to albumin ratio, a relatively new marker of inflammation, and the development of postoperative atrial fibrillation. Materials and Method: 418 patients who underwent coronary artery surgery between 2017‐2019 were included in the study. While 88 patients with postoperative atrial fibrillation were in group 1, 330 patients with atrial fibrillation were studied in group 2. The relation‐ ship between the C‐reactive protein/albumin ratio determined from serum samples taken before the operation and the development of postoperative atrial fibrillation was evaluated retrospectively. Results: C‐reactive protein/albumin ratio was significantly higher in patients with postoperative atrial fibrillation. In multivariate re‐ gression analysis evaluation, C‐reactive protein/albumin ratio was an independent variable in postoperative atrial fibrillation formation. The predictive of CAR, c reactive protein, and albumin was compared by ROC curve analysis. C‐reactive protein/albumin ratio, C‐ reac‐ tive protein and albumin were compared with ROC analysis in terms of predictive valence. C‐reactive protein/albumin ratio was ob‐ served as a more sensitive and stronger predictive parameter than C‐ reactive protein and albumin. Conclusion: The use of the C‐reactive protein/albumin ratio as a potent inflammatory marker has become increasingly popular in recent years. The correlation between pre‐operative inflammation findings and postoperative atrial fibrillation has long been known. With this study, it has been shown that the power of this proportional value in predicting the risk of postoperative atrial fibrillation is a more re‐ liable parameter compared to only C‐reactive protein or albumin alone.

Özet

Amaç: Atriyal fibrilasyon, kalp cerrahisi sonrası en zorlu postoperatif komplikasyonlardan biridir. Postoperatif atriyal fibrilasyon oluş‐ması morbidite ve mortaliteyi artırdığı gibi sağlık harcamalarını da artırmaktadır. Bu nedenle risk grubundaki hastaların belirlenmesi ve önlem alınması önemlidir. Ameliyat öncesi atriyal fibrilasyonu tetikleyen enflamatuvar durumun belirlenmesi de bu hastaların saptan‐masına önemli katkı sağlar. Bu çalışmada nispeten yeni bir enflamasyon belirteci olan C‐reaktif protein/albümin oranı ile postoperatif atriayal fibrilasyon gelişimi arasındaki ilişkinin incelenmesi amaçlanmıştır. Gereç ve Yöntem: 2017‐2019 yılları arasında koroner arter cerrahisi geçiren 418 hasta çalışmaya dahil edildi. Operasyon sonrası atrial fibrilasyon tespit edilen 88 hasta grup 1’de, atriyal fibrilasyonla izlenen 330 hasta grup 2 içerisinde incelendi. Operasyon öncesi alınan serum örneklerinden tespit edilen C‐reaktif protein/albümin oranı ile postoperatif atriyal fibrilasyon gelişimi arasındaki ilişki retrospek‐tif olarak değerlendirildi. Bulgular: C‐reaktif protein/albümin oranı postoperatif atriyal fibrilasyonu olan hastalarda anlamlı olarak daha yüksekti (4,4±0,65'e kar‐şı 2,7±0,29, p=0,039). Çok değişkenli regresyon analizi değerlendirmesinde, C‐reaktif protein/albümin oranı, postoperatif atriyal fibri‐lasyon oluşumunda bağımsız bir değişken olarak saptandı (OR=1,53 (0,45‐2,76), p=0,019). ROC eğrisi analizinde C‐reaktif protein/albü‐min oranı, C‐reaktif protein ve albümine göre daha duyarlı ve daha güçlü bir öngörü parametresi olarak gözlendi. Sonuç: C‐reaktif protein/albümin oranının güçlü bir enflamatuvar belirteç olarak kullanımı son yıllarda giderek daha popüler hale gel‐miştir. Operasyon öncesi izlenen enflamasyon bulguları ile postoperatif atriyal fibrilasyon arasındaki korelasyon ilişkisi öteden beri bi‐linmektedir. Bu çalışma ile bu oransal değerin postoperatif atriyal fibrilasyon riskini öngörmedeki gücünün, sadece C‐reaktif protein ve‐ya sadece albümine kıyasla daha güvenilir bir parametre olduğu gösterilmiştir.
