The value of neuropsychological tests in demonstrating lateralization: A case report

Abstract
A twenty-seven-year-old man presented to the neurology outpatient clinic with complaints of discomfort, concentration defects, and pensiveness. There was no abnormality on neurological examination. Electroencephalography showed sharp-waves, spikewaves, and 3-5 Hz sharp theta activities in the right hemisphere and left frontal area. Cranial magnetic resonance imaging showed enlarged sulcuses being more prominent in the left frontal lobe and an arachnoid cyst in the medial part of the temporal lobe. Psychiatric examination suggested dissociative problems and depersonalization disorder. Temporal lobe dysfunctions and impairment in the right hemisphere were detected by neuropsychological tests. A close correlation was found between neuropsychological examination and functional investigations.
Özet
Yirmi yedi yaşındaki erkek hasta kendini kötü hissetme, konsantre olamama ve dalma şikayetleriyle nöroloji polikliniğine başvurdu. Nörolojik muayenesinde belirgin patoloji saptanmadı. Elektroensefalografi incelemesinde sağ hemisferde ve sol frontalde sürekli keskin dalgalar, diken dalgalar, 3-5 cy/sn'lik keskin teta aktiviteleri gözlendi. Kranyal manyetik rezonans görüntüleme incelemesinde solda daha belirgin olmak üzere frontal lob sulkuslarında hafif genişleme ve temporal lob medialinde araknoid kist saptandı. Tam bir psikiyatrik muayene ile değerlendirilen olguda, disosiyatif bozukluk ve kişilik bozukluğu tanıları kondu. Nöropsikolojik değerlendirmelerde temporal lob işlevlerinde kayıp ve sağ hemisfer bozukluğu saptandı. Nöropsikolojik testlerle fonksiyonel araştırmalar sonucu elde edilen bulguların uyumluluğu görüldü.
