Comparison of suture and fibrin adhesive techniques in microvascular anastomosis of rat carotid arteries

Abstract

Objectives: It is well known that suture material used in microvascular anastomosis leads to foreign body reaction and necrosis in the tunica intima and media of the artery. Therefore, new techniques have been developed in microvascular anastomosis in which fibrin glue is used and the number of sutures are reduced. Study Design and Methods: We employed end-to-end anastomosis via three different techniques on the carotid arteries of 33 Wistar albino rats, namely the classic suture technique (9 sutures), sleeve technique (2 sutures) using fibrin glue, and end-to-end anastomosis (2 sutures) with fibrin glue. Results: The most favorable histopathological results were obtained using end-to-end anastomosis with fibrin glue. Other advantages were its simplicity requiring a shorter time. However, with this technique, rupture occurred in two of the 11 arteries. Conversely, the sleeve technique using fibrin glue yielded no ruptures. Conclusion: Combination of suture techniques with fibrin glue seems to influence outcome favorably. Ruptures occurring with end-to-end anastomosis using fibrin glue may cast suspicion on the adequacy of stability obtained with this technique.

Özet

Amaç: Mikrovasküler anastomozlarda kullanılan dikiş materyallerinin yabancı doku reaksiyonu ve damarın intima ile media tabakalarında nekroza neden olduğu bilinmektedir. Bu nedenle, mikrovasküler anastomozda fibrin yapıştırıcıdan yararlanarak dikiş sayısını azaltmaya yönelik yeni teknikler ortaya konmuştur. Çalışma Planı ve Yöntemler: Çalışmamızda 33 adet Wistar albino sıçan karotid arteri üzerinde üç değişik teknikle uç uca anastomoz uyguladık: Klasik dikiş tekniği (9 dikiş), fibrin yapıştırıcı kullanılan iki dikişli teleskop (iç içe) tekniği ve uç uca anastomoz tekniği. Bulgular: En iyi histopatolojik sonucu fibrin yapıştırıcı kullandığımız uç uca anastomoz tekniği ile elde ettik. Bu tekniği ön plana çıkaran diğer avantajları ise tekniğin kolaylığı ve uygulama süresinin kısa oluşudur. Ancak bu teknikte 11 arterden ikisinde rüptür gözlendi. Fibrin yapıştırıcılı teleskop tekniğinde ise rüptüre rastlanmadı. Sonuç: Fibrin yapıştırıcının dikiş teknikleriyle kombine edilmesi, sonuçları olumlu yönde etkilemektedir. Fibrin yapıştırıcılı uç uca anastomoz tekniğinde gözlenen rüptürler, bu teknikle anastomozun yeterli stabiliteyi sağlayamadığını düşündürdü.
